摘  要:目的探讨降钙素原(PCT)联合C-反应蛋白(CRP)检测对血液病患者病原菌感染的临床诊断意义,为血液病患者的用药提供依据。方法选取2011年5月-2013年5月医院治疗的48例血液病感染患者为研究对象,将患者根据所感染的病原菌种类分为3组,其中革兰阳性菌感染组17例、革兰阴性菌感染组15例、真菌感染组16例;对比3组患者血清中PCT水平和CRP水平,同时比较单独使用CRP检测与PCT联合CRP检测的特异性和敏感性;采用SPSS16.0软件进行统计分析。结果在革兰阴性菌感染组中,检测到的血清中PCT水平和CRP水平分别为(11.34±23.14)μg/L、(133.87±94.61)mg/L,革兰阳性菌感染组中PCT水平和CRP水平分别为(5.39±20.04)μg/L、(94.37±75.36)mg/L,真菌感染组中检测到的PCT水平和CRP水平分别为(0.41±2.31)μg/L、(142.51±42.38)mg/L,革兰阴性菌、革兰阳性菌感染组的PCT水平比真菌菌感染组的明显升高,差异有统计学意义(P<0.05),单独只做CRP水平的检测,其特异性和敏感性分别为68.31%和88.79%,通过PCT联合CRP的检测其特异性和敏感性依次为80.1%和85.3%。结论 PCT联合CRP检测能给血液病患者病原菌感染的诊断提供重要依据,为临床用药提供指导。OBJECTIVE To explore the clinical significance of procalcitonin(PCT)combined with C-reactive protein(CRP)in diagnosis of infections in patients with blood diseases so as to provide guidance for use of antibiotics.METHODS A total of 48 blood diseases patients with infections who were treated in the hospital from May 2011 to May 2013 were recruited as the study objects and divided into three groups according to the species of pathogens causing the infections:the gram-positive bacteria infection group(17cases),the gram-negative bacteria infection group(15cases),and the fungal infection group(16cases).The levels of serum PCT and CRP were compared among the three groups of patients,the specificity and sensitivity in detection of the infections were observed and compared between the single use of CRP and the PCT combined with CRP,and the statistical analysis was performed with the use of SPSS16.0software.RESULTS The levels of serum PCT and CRP of the gram-negative bacteria infection group were(11.34±23.14)μg/L and(133.87±94.61)mg/L,respectively;the levels of PCT and CRP of the gram-positive bacteria infection group were(5.39±20.04)μg/L and(94.37±75.36)mg/L,respectively;the levels of PCT and CRP of the fungal infection group were(0.41±2.31)μg/L and(142.51±42.38)mg/L,respectively;the level of PCT was significantly higher in the gram-negative bacteria infection group and the gram-positive bacteria infection group than in the fungal infection group.The specificity of the single detection of CRP was 68.31%,the sensitivity 88.79%;the specificity of the detection of PCT combined with CRP was80.1%,the sensitivity 85.3%.CONCLUSIONThe PCT combined with CRP detection has great significance in diagnosis of pathogenic bacteria infections in the patients with blood diseases and may provide guidance for clinical use of antibiotics.
