开放性骨折后骨外固定闭式引流治疗骨感染的临床研究

摘　　要：目的研究骨外固定支架结合负压封闭引流(VSD)在开放性骨折后骨感染患者中的临床治疗效果,为提高临床治愈率提供依据。方法选取2014年12月-2015年11月医院诊治的66例开放性骨折后感染患者,随机分为对照组和试验组,每组各33例;对照组采用骨外固定治疗,试验组在对照组基础上联合VSD治疗,对两组患者治疗后的临床指标进行比较;采用SPSS18.0软件进行统计分析。结果试验组患者治疗后住院时间、外固定拆除时间以及功能康复时间,显著短于对照组(P<0.05);试验组患者治疗后总有效率为93.9%,显著高于对照组的78.8%(P<0.05);试验组患者治疗后不良反应发生率为12.1%,显著低于对照组的33.3%(P<0.05)。结论开放性骨折后感染患者在骨外固定治疗基础上联合VSD治疗效果理想,能有效提高临床疗效,并发症发生率低,值得推广应用。OBJECTIVE To observe the clinical effect of bone external fixation combined with vacuum sealing drainage（VSD）on treatment of open facture patients with bone infections so as to raise the clinical cure rate.METHODS A total of 66 open fracture patients with infections who were treated in the hospital from Dec 2014 to Nov 2015 were enrolled in the study and randomly divided into the control group and the experimental group,with33 cases in each.The control group was treated with bone external fixation,while the experimental group was given the bone external fixation combined with VSD.The clinical indicators were observed and compared between the two groups of patients after the treatment,and the statistical analysis was performed with the use of SPSS18.0software.RESULTS The length of hospital stay,time of removal of external fixtor,and time of functional recovery were significantly shorter in the experimental group than in the control group after the treatment（P〈0.05）.The total effective rate of treatment of the experimental group was 93.9%after the treatment,significantly higher than78.8% of the control group（P〈0.05）.The incidence of adverse reactions of the experimental group was 12.1%after the treatment,significantly lower than 33.3% of the control group（P〈0.05）.CONCLUSIONThe bone external fixation combined with VSD can achieve ideal effect on treatment of the open fracture patients with bone infections,effectively improve the clinical effect,and reduce the incidence of complications;it is worthy to be promoted.
