摘 要:目的探讨封闭式负压引流术(VSD)在手外伤感染创面修复中的治疗效果,为临床感染的创面修复提供技术参考。方法选择2013年6月-2015年2月医院接诊的手外伤感染患者68例,随机将患者分为观察组和对照组,各34例,观察组接受VSD治疗,而对照组接受传统每日换药处理,统计两组患者的抗菌药物使用时间、创面愈合时间、住院时间、二次手术率、二期植皮时间和组织病理学评分等,并评价创面修复效果;采用SPSS 19.0软件进行统计分析。结果观察组患者抗感染药物使用时间、创面愈合时间和住院时间分别为(7.1±1.2)d、(15.8±1.6)d和(18.9±2.1)d,对照组为(13.4±1.5)d、(26.8±2.1)d和(28.5±3.4)d,观察组患者抗感染药物使用时间、创面愈合时间和住院时间均明显短于对照组(P<0.05);观察组患者二次手术率、二期植皮时间明显小于对照组,而组织病理学评分明显高于对照组(P<0.05);观察组患者有效率94.12%、对照组为73.53%,观察组有效率明显高于对照组(P<0.05)。结论 VSD应用于手外伤感染创面修复效果显著,且能明显缩短抗感染药物使用时间和住院时间,促进创面愈合。OBJECTIVE To explore the effect of vacuum sealing drainage(VSD)on wound repair of patients with hand injury infections so as to provide guidance for wound repair of the patients with infections.METHODS A total of 68 patients with hand injury infections who were treated in hospitals from Jun 2013 to Feb 2015 were enrolled in the study and randomly divided into the observation group and the control group,with 34 cases in each.The observation group was treated with VSD,while the control group was given the traditional daily drug dressing.The time of use of antibiotics,wound healing time,length of hospital stay,rate of secondary operation,time of phase II skin grafting,and histopathological score were taken for statistics,the effect of wound repair was evaluated,and the statistical analysis was performed with the use of SPSS19.0software.RESULTS The time of use of antibiotics of the observation group was(7.1±1.2)days,significantly shorter than(13.4±1.5)days of the control group;the wound healing time of the observation group was(15.8±1.6)days,significantly shorter than(26.8±2.1)days of the control group;the length of hospital stay of the observation group was(18.9±2.1)days,significantly shorter than(28.5±3.4)days of the control group(P〈0.05).The rate of secondary operation of the observation group was significantly lower than that of the control group,the time of phase II skin grafting of the observation group was significantly shorter than that of the control group,however,the histopathological score of the observation group was significantly higher than that of the control group(P〈0.05).The effective rate of the observation group was 94.12%,significantly higher than 73.53% of the control group(P〈0.05).CONCLUSION The VSD can achieve significant effect on wound repair of the patients with hand injury infections,remarkably shorten the time of use of antibiotics and length of hospital stay,and promote the wound healing.
