摘  要:目的通过对耐碳青霉烯类鲍氏不动杆菌的分子流行病学调查,了解耐碳青霉烯类鲍氏不动杆菌的基因型分布和传播途径,为有效预防控制提供依据。方法收集某三级医院2011年1月-2012年12月分离自住院患者痰标本的耐碳青霉烯类鲍氏不动杆菌212株;采用PFGE和MLST方法对其进行分子流行病学分析;149株获得PFGE可分析结果,184株获得MLST可分析性结果。结果经PFGE方法获得的结果显示,B型菌株为优势菌株,102株占68.46%,B型菌株中的67株分布在ICU;MLST分型结果显示,ST195占优势(32.6%),其次为ST208(23.4%)和ST643(17.9%),并发现15株新的ST克隆株型别(8.2%)。结论 PFGE是区域内分析细菌传播途径的有效方法,本研究中院内流行的耐碳青霉烯类鲍氏不动杆菌主要来自于ICU;MLST是全球范围内比对细菌克隆菌株差异的好工具,全球鲍氏不动杆菌的克隆菌株分布差异较大,新的克隆菌株不断出现,提示其变异和演变的速度较快。OBJECTIVE To investigate the distribution of genotypes and transmission routes of carbapenem-resistant Acinetobacter baumannii(CRAB)through molecular epidemiological investigation of CRAB so as to provide guidance for effective prevention and control.METHODS A total of 212 strains of CRAB were isolated from sputum specimens of the patients who were hospitalized a tertiary care hospital from Jan 2011 to Dec 2012,then the molecular epidemiological analysis was carried out for the strains by using PFGE and MLST methods,149 strains obtained PFGE analysis results,and 184 strains obtained MLST analysis results.RESULTS The analysis of the strains that were obtained by PFGE showed that the genotype B strains were dominant,accounting for 68.46%(102strains),and 67 genotype B strains distributed in the ICU.The results of MLST genotyping indicated that the ST195(32.6%)was dominant,followed by the ST208(23.4%)and ST643(17.9%),and totally 15(8.2%)strains of novel ST cloned types were found.CONCLUSIONThe PFGE is an effective method for regional analysis of bacterial transmission,the CRAB strains that are prevalent in the hospitals are mainly isolated from the ICU.The MLST is a good tool to distinguish the cloned strains worldwide,the clone strains of A.baumannii vary significantly in the global distribution,novel clone strains emerge constantly,indicating the variation and evolution are rapid.
