摘 要:目的以微量肉汤稀释法为参考方法,评估商品化Sensititre YeastOne显色药敏板条检测念珠菌属体外药物敏感性的临床应用价值。方法分别用微量肉汤稀释法和YeastOne显色药敏板条同时检测111株念珠菌属对临床常用7种抗真菌药物的体外敏感性,孵育时间为24h;以微量肉汤稀释法作为参考方法,YeastOne为试验方法进行对照分析。结果 YeastOne与微量肉汤稀释法的MIC值一致率高低与所检测的念珠菌属及抗真菌药物种类有关;总一致率:氟康唑77.3%~100.0%、伏立康唑77.3%~100.0%、伊曲康唑45.5%~95.5%、两性霉素B 95.5%~100.0%、氟胞嘧啶98.2%~100.0%、卡泊芬净61.1%~100.0%、米卡芬净72.7%~100.0%;对于伊曲康唑,YeastOne方法检测白色念珠菌和近平滑念珠菌体外敏感性时,与微量肉汤稀释法比较存在较大差异;对卡泊芬净,YeastOne方法检测白色念珠菌和热带念珠菌时与微量肉汤稀释法比较存在差异;其他药物两种方法一致性较好;YeastOne方法和微量肉汤稀释法相比,未发现极严重错误(VME)和差异有本质区别(SD)。结论相对于微量肉汤稀释法,Sensititre YeastOne检测念珠菌属体外药物敏感性操作简便,结果判读客观,易于推广应用,作为临床微生物实验室抗真菌药物敏感性试验的检测方法具有较好的实用价值。OBJECTIVE To evaluate the clinical value of the commercialized Sensititre YeastOne chromogenic susceptibility panel in detection of in vitro drug susceptibility of Candidaby setting the broth microdilution(BMD)as the reference method.METHODS The in vitro susceptibilities of 111 strains of Candida sppto 7commonly used antifungal drugs were detected by respectively using the BMD method and YeastOne chromogenic susceptibility panel.The BMD method was adopted as reference method,the YeastOne was as test method;the reference BMD and YeastOne colorimetric MIC endpoints were read after 24 hof incubation.RESULTS The concordance rate of MIC between the reference BMD and Sensititre YeastOne for each agent was associated with the species of Candidaand the types of antifungal drugs.The overall concordance rate for fluconazole was from 77.3% to 100.0%,voriconazole(77.3%-100.0%),itraconazole(45.5%-95.5%),amphotericin B(95.5%-100.0%),flucytosine(98.2%-100.0%),caspofungin(61.1%-100.0%),micafungin(72.7%-100.0%).There was significant difference in the detection of in vitro susceptibility of Candida albicans and Candida parapsilosis to itraconazole between the YeastOne method and BMD method;there was difference in the detection of in vitro susceptibility of C.albicans and C.tropicalis to caspofungin between the YeastOne method and BMD method;the two methods were highly consistent in detection of in vitro susceptibility to other antibiotics.None of very major error(VME)and substantial differences(SD)were determined by the YeastOne method compared to the reference BMD method.CONCLUSION As compared with the BMD method,the Sensititre YeastOne method is a simple and convenient method for the detection of in vitro drug susceptibility of the Candidaspecies,with the detection results objective,it is convenient to be applied and has significant practical value in the detection of susceptibility to the antifungal drugs in clinical microbiology laboratory.
