尖锐湿疣感染患者不同病程T淋巴细胞亚群的表达与人乳头状瘤病毒感染的相关性研究

摘　　要：目的分析尖锐湿疣初发、复发、不同病程患者间T淋巴细胞亚群的表达及人乳头状瘤病毒(HPV)感染与其相关性,为临床诊治提供参考依据。方法选取2014年1月-2015年4月医院收治尖锐湿疣患者72例为研究对象,初发患者34例为甲组,复发患者38例为乙组,另外选取同期健康体检人员50名为对照组,检测各组受试人员外周血T淋巴细胞亚群,分析尖锐湿疣患者HPV阳性率,并分析HPV感染类型外周T淋巴细胞亚群表达。结果乙组患者CD_4^+、CD_4^+/CD_8^+显著低于对照组和甲组(P<0.05),CD8+显著高于对照组和甲组(P<0.05);经PCR检测,患者均呈现HPV阳性,其中单纯HPV6/11型感染22例,单纯HPV16/18感染23例,混合感染27例,3组受试人员T淋巴细胞亚群差异无统计学意义。结论尖锐湿疣患者细胞免疫功能下降,病程越长,免疫功能下降更加明显,HPV感染免疫功能低下更加容易引起尖锐湿疣的复发。OBJECTIVE To explore the correlation between the expression of T lymphocyte subsets of patients with primary or recurrent condyloma and the human papillomavirus（HPV）infection so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 72 patients with condyloma who were treated in the hospital from Jan 2014 to Apr 2015 were recruited as the study objects,34 patients with primary condyloma were assigned as the group A,and 38 patients with recurrent condyloma were set as the group B;meanwhile,50 healthy people who received the physical examination were chosen as the control group.The peripheral blood T lymphocyte subsets were detected for the subjects,the positive rate of HPV in the patients with condyloma was analyzed,and the expression of peripheral blood T lymphocyte subsets in the patients with different types of HPV infections was observed.RESULTS The levels of CD_4＋and CD_4＋/CD_8＋were significantly lower in the group B than in the control group and the group A（P〈0.05）;the level of CD8＋was significantly higher in the group B than in the control group and the group A（P〈0.05）.The PCR examination showed that all the patients were tested positive for HPV,of whom 22 had single HPV6/11 infection,23had single HPV16/18 infection,and 27 had the mixed infections.There was no significant difference in the T lymphocyte subsets among the three groups of subjects.CONCLUSIONThe cellular immune function of the patients with condyloma is declined with the course of disease,and the HPV infection tends to result in the recurrence of condyloma.
