慢性肾衰竭患者合并急性肾损伤感染的临床危险因素分析

摘　　要：目的探讨析慢性肾衰竭合并急性肾损伤感染的临床相关危险因素,为临床诊断治疗提供依据。方法选取2012年6月-2014年6月医院收治的慢性肾衰竭合并急性肾损伤感染患者123例为研究对象,回顾性分析患者临床资料,了解患者发生慢性肾衰竭合并急性肾损伤感染的相关危险因素;采用单因素分析方法初步选出影响患者预后的危险因素,并对初选结果中有统计学意义的相关因素作为自变量进行logistic回归分析,了解影响患者预后的相关因素及提出相应预防措施;采用SPSS16.0软件进行统计分析。结果诱发慢性肾衰竭合并急性肾损伤感染的前3位危险因素为高龄、药物性的肾损伤以及恶性高血压;患者容易出现此疾病的前3位基础肾脏性疾病依次为糖尿病肾病、慢性肾小球肾炎以及高血压性肾病;logistic回归分析结果显示,多脏器功能障碍综合征(MODS)器官衰竭数目、高血钾以及血红蛋白低是影响患者预后的相关因素(P<0.05)。结论对于慢性肾衰竭合并急性肾损伤感染患者,控制好患者的血压和血糖,避免发生感染,避免使用肾毒性的药物,才能有效的治疗原发疾病,预防慢性肾衰竭合并急性肾损伤感染的发生,有效延缓慢性肾衰竭的发展。OBJECTIVE To explore the clinical risk factors for acute kidney injury infections in chronic renal failure patients so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 123 chronic renal failure patients complicated with acute kidney injury infections who were treated in the hospital from Jun 2012 to Jun2014were recruited as the study objects,the clinical data of the patients were retrospectively analyzed,and the related risk factors for the acute kidney injury infections in the chronic renal failure patients were investigated.The influencing factors for prognosis of the patients were preliminarily screened out through univariate analysis,the logistic regression analysis was carried out for the related factors with statistical significance,which were set as the independent variables.The influencing factors for prognosis of the patients were investigated,the corresponding prevention measures were put forward,and the statistical analysis was performed with the use of SPSS16.0software.RESULTS The top 3risk factors for the acute kidney injury infections in the chronic renal failure patients were as follows：the advanced age,drug-induced kidney injury,and malignant hypertension.The top 3underlying renal diseases that tended to occur in the patients were in turn as follows：diabetic nephropathy,chronic glomerulonephritis nephritis,and hypertensive renal disease.The results of the logistic regression analysis indicated that the number of organ failure of multiple organ dysfunction syndrome（MODS）,hyperkalemia,and low level of hemoglobin were the influencing factors for prognosis of the patients（P〈0.05）.CONCLUSIONFor the chronic renal failure patients complicated with acute kidney injury infections,it is an effective way to control the blood pressure and blood glucose to prevent the infections and avoid the application of nephrotoxic drugs so as to treat the primary diseases,prevent the chronic renal failure complicated with acute kidney injury infections,and delay the development
