封闭式负压引流术治疗人工关节置换术后深部感染的临床研究

摘　　要：目的探讨封闭式负压引流术(VSD)治疗人工关节置换术(THA)后深部感染的效果,为临床治疗提供依据。方法选择2012年12月-2014年12月于医院接受THA并发深部感染的患者96例,随机将其分为观察组和对照组,各48例;观察组患者接受VAD治疗,对照组患者接受传统持续灌注冲洗治疗,统计两组患者的感染控制时间、创面清洁时间、住院时间、C-反应蛋白(CRP)水平、体温和McGill疼痛评分等,并评价感染控制效果,数据采用SPSS 19.0统计软件进行分析。结果观察组患者感染控制、创面清洁和住院时间均明显短于对照组,差异有统计学意义(P<0.05);术后3d,观察组患者CRP、体温和McGill疼痛评分均明显低于对照组,差异有统计学意义(P<0.05);观察组患者治疗有效率为93.8%,明显高于对照组的75.0%,差异有统计学意义(P<0.05)。结论 VSD应用于THA术后深部感染控制效果显著,且能明显缩短感染控制和住院时间,促进创面愈合,为临床骨科治疗术后感染提供新的技术参考。OBJECTIVE To observe the efficacy of vacuum sealing drainage（VSD）on deep infection prevention in patients after total hip replacement（THA）.METHODS Totally 96 patients with deep infection after THA surgery in the hospital from Dec.2012 to Dec.2014 were selected.Patients were randomly divided into the observation group and the control group,48 cases in each group.The observation group was treated with VSD,while the control group accepted traditional continuous perfusion washing.Infection control time,wound cleaning time,hospitalization time,C-reactive protein levels（CRP）,body temperature and McGill pain score as well as infection control effect were observed and compared between two groups.RESULTS Infection control time,wound cleaning time,and hospitalization time were significantly less in the observation group than the control group（P〈0.05）.At three days after surgery,CRP,body temperature and McGill pain scores in the observation group were significantly less than in the control group（P〈0.05）.The efficient rate was 93.8%in the observation group,significantly higher than 75.0%in the control group（P〈0.05）.CONCLUSIONThe control effect of deep infection after THA by VSD was significant,and it can significantly shorten the time of infection control and hospital stay,promote wound healing,and provide a new technical reference for the clinical treatment of VSD.
