摘 要:目的评价社区卫生服务中心口腔科的复用器械强化预处理清洗流程的效果,以提高器械消毒灭菌质量。方法收集2014年5月20日-6月20日北京市西城区新街口社区卫生服务中心口腔科各类牙科手术器械30件,按照不同的清洗流程将其分为对照组和试验组,各15件;对照组采用传统的处理方法,器械使用后先在多酶液中浸泡,然后使用超声波加酶液震荡清洗,再用清水漂洗;试验组采用改进方法,器械使用后先用多酶湿纸巾及时对器械进行擦拭,有效防止污渍干涸,然后进行手工清洗加超声波清水震荡,再水漂洗;对两组器械的清洗效果进行进行对比分析。结果试验组和对照组器械使用后,肉眼、细菌学及ATP检测阳性率,差异均无统计学意义;两组器械预处理后肉眼、ATP测量合格率的差异无统计学意义;试验组细菌学检测合格率66.67%,高于对照组的26.67%,差异有统计学意义(χ2=4.821,P<0.05);试验组器械ATP、细菌学检测合格率分别为66.67%、86.67%,均高于对照组的20.00%、26.67%,差异均有统计学意义(χ2=10.995、P<0.05)。结论试验组的方法对口腔复用器械预处理的清洗流程更有效,显著提高了器械消毒灭菌的效果,杜绝了由器械不洁造成口腔感染的风险。OBJECTIVE To evaluate the effect of enhanced preprocessing of cleaning procedure for reusable instruments in community health service centers so as to improve the quality of disinfection and sterilization of the instruments.METHODS A total of 30 pieces of dental surgical instruments were collected from Beijing Xicheng District Xinjiekou Community Health Service Center from May 20,2014 to Jun 20,2014 and were divided into the control group and the experimental group,with 15 pieces in each.The control group was treated with conventional processing method,the instruments were soaked in multienzyme solution,cleaned with ultrasonic wave plus enzyme solution,and rinsed with water;the experimental group was given the modified method,the instruments were wiped with multienzyme wet wipes in a timely manner to keep the stains wet,treated with manual cleaning plus ultrasonic wave,and rinsed with water.The cleaning effects were observed and compared between the two groups of instruments.RESULTS There was no significant difference in the positive rate of visual observation,bacteriological detection,or ATP detection between the experimental group and the control group after the instruments were used.There was no significant difference in the qualified rate of visual observation or ATP detection between the two groups after the preprocessing;the qualified rate of bacteriological detection of the experimental group was66.67%,significantly higher than 26.67% of the control group(χ2=4.821,P〈0.05).The qualified rate of ATP detection of the experimental group was 66.67%,higher than 20.00% of the control group;the qualified rate of bacteriological detection of the experimental group was 86.67%,higher than 26.67% of the control group,and there was significant difference(χ2=10.995,P〈0.05).CONCLUSION The method of the experimental group is more effective for the preprocessing cleaning procedure of the reusable dental instruments,remarkably improve the effect of disinfection and sterilization,and prevent the oral infections
