TLR-2与TLR-4在胎膜早破患者胎盘组织中的表达及意义

摘　　要：目的探讨Toll样受体2(TLR-2)与TLR-4在胎膜早破患者胎盘组织中的表达及意义。方法选取2014年6月至2015年5月淮安市妇幼保健院产科分娩的100例足月胎膜早破患者列入足月胎膜早破组,100例早产胎膜早破患者(<37周)列入早产胎膜早破组,上述两组共同纳入胎膜早破组,100例足月正常分娩孕妇列入对照组。取受试患者分娩后的胎膜破口处组织为化验样本苏木精-伊红染色和免疫组织化学试验,观察绒毛膜羊膜炎发生率和胎膜组织TLR-2、TLR-4蛋白表达情况。结果胎膜早破组患者绒毛膜羊膜炎发生率显著高于对照组[48.0%(96/200)比8.0%(8/100),P<0.01],但足月胎膜早破组、早产胎膜早破组比较差异无统计学意义(P>0.05)。胎膜早破组患者TR-2与TLR-4表达数显著高于对照组[(11.01±2.13)阳性细胞数/mm2比(8.01±1.78)阳性细胞数/mm2,(12.29±3.22)阳性细胞数/mm2比(6.17±1.36)阳性细胞数/mm2](P<0.01),但足月胎膜早破组和早产胎膜早破组间比较差异无统计学意义(P>0.05)。结论 TLR-2与TLR-4可通过免疫介导作用参与胎膜早破的发展过程,对两种因子的深入了解将有利于对胎膜早破的早期预防研究,两者有望成为胎膜早破免疫治疗的新靶位。Objective To discuss the expression and significance of Toll like receptor （TLR）-2 and TLR-4 in the placenta of patients with premature rupture of membranes. Methods Total of 100 cases of full term delivery in the Department of Obstetrics in Huai＇an Maternal and Child Health-Care Center from Jun. 2014 to May 2015 were included as full term premature membrane rupture group,and 100 cases of preterm with premature rupture of membranes（ 〈 37 weeks） were included as preterm premature membrane rupture group ,while 100 cases of full-term normal delivery were included as control group. After delivery fetal mem- brane rupture tissue were taken for laboratory HE staining and immunohistochemistry test, chorioamnionitis incidence and TLR-2 and TLR-4 protein expression in fetal tissue were observed. Results The ehofioamnio- nitis incidence of premature membrane rupture group was significantly higher than the control group [48.0% （96/200） vs 8.0% （8/100） ] （P 〈0. 01 ） ,but full-term premature membrane rupture group and preterm premature membrane rupture group had no statistical difference （P 〉 0. 05 ）. TLR-2 and TLR-4 expression of premature membrane rupture groups were significantly higher than the control group[ （11.01 ± 2. 13 ） posi- tive cells/mm2 vs （ 8.01 ± 1.78 ） positive cells/mm2, （ 12. 29 ± 3.22 ） positive cells/ram2 vs （ 6. 17 ± 1.36 ） positive cells/mm2] （P 〈 0. 01 ）, but the difference between full term and preterm premature mem- brane rupture groups was not significant （ P 〉 0. 05 ）. Conclusion TLR-2 and TLR-4 are involved in prema- ture rupture of membranes process through immune mediated action, so in-depth understanding of the two fac- tors will help for the early prevention research of premature rupture of membranes, both of which are expected to become new targets for the immunotherapy.
