Ca125、HCG、P对早期先兆流产患者妊娠结局预测的比较

摘　　要：目的：研究妊娠第5-7周血清糖链蛋白（carbohydrate antigen,Ca）125、人绒毛膜促性腺激素（human chorionic gonadotropin,HCG）和孕酮（progesterone,P）水平预测早期先兆流产患者妊娠结局的价值。方法：按纳入、排除标准选取2013年1月至2013年12月就诊于江西省中医院的妊娠5-7周先兆流产患者267例,前来我院早孕体检者96例。按妊娠结局分为继续妊娠组及难免流产组,早孕体检者为正常妊娠组。先兆流产患者均在接受治疗前检测Ca125、HCG、P水平,随访所有病例至妊娠12周,分析各组的Ca125、HCG、P水平的变化,以及对早期先兆流产妊娠结局预测灵敏度的差异。结果：1正常妊娠组、继续妊娠组血清Ca125水平高于难免流产组。组间差异有统计学意义（χ2=61.950,P=0.000）;2妊娠5-7周,Ca125诊断难免流产的灵敏度77.4%,特异度80.0%。HCG诊断难免流产的灵敏度75.3%,特异度68.5%。P诊断难免流产的灵敏度76.3%,特异度68.5%。结论：妊娠5-7周Ca125、HCG和P在预测难免流产中均有较高的诊断价值。Objective：To research the value of carbohydrate antigen 125 （Ca125）,human chorionic gonadotropin（HCG） and proges- terone（P） value in the prediction of pregnancy outcome of patients with threatened abortion at early stage. Methods：Totally 267 pa- tients with threatened abortion at 5-7 weeks of gestation in Jiangxi Traditional Chinese Medicine Hospital from January 2013 to De eember 2013 were selected according to inclusion and exclusion criteria. Ninety-six patients with early pregnancy were divided into continued pregnancy group and inevitable abortion group by pregnancy result, while the early pregnancy testers were taken as normal pregnancy group. Ca125, HCG and P levels of patients with early pregnancy were detected and follow-up was condueted till the 12th week of pregnancy. Sensitivity and specificity of Ca125, HCG, and P in the diagnosis of threatened abortion were analyzed. Results：Levels of Ca125 were higher in normal pregnancy group and continued pregnancy group than in inevitable abortion group, with significant differences among groups （P〈0.001）At 5-7 weeks of gestation, the sensitivity of Ca125 in the diagnosis of inevitable abortion was 76.3% and specificity was 68.5%. Conc/us/on：Ca125 ,HCG and P have high diagnostic value in the prediction of in- evitable abortion.
