缅甸籍跨境女性性工作者苯丙胺类兴奋剂使用与高危性行为的相关性分析

摘　　要：目的 探索缅甸籍跨境女性性工作者苯丙胺类兴奋剂（Amphetamine-Type Stimulant,ATS）使用与高危性行为之间的关系。方法 2013年3月至4月,在云南省德宏州瑞丽市姐告和缅甸木姐口岸,采用雪球抽样方式纳入147名调查对象。自行设计问卷进行横断面调查,内容包括人口学特征、ATS使用情况、性行为特征等。同时,对调查对象进行HIV血检和甲基安非他明尿检。结果 甲基安非他明尿检阳性者46人,使用的ATS主要为冰毒和摇头丸。23人（50.00%）曾在服用ATS后与客人发生性行为且没有坚持使用安全套。ATS尿检阳性（OR=3.362,95%CI：1.625~6.956）和ATS药效期间与客人发生性行为（OR=1.911,95%CI：1.007~3.625）是促进缅甸籍跨境女性性工作者发生高危性行为的危险因素。结论 针对缅甸籍跨境女性性工作者ATS使用的相关干预措施应当成为今后中缅边境地区该人群行为干预工作的重点,同时,应当继续加强安全套使用的宣传教育和行为干预工作。Objective This work was to explore the correlation between Amphetamine -type stimulants use and high- risk sexual behavior of Myanmar female sex workers （FSWs）. Methods During March to April in 2013 at JieGao city of DeHong state,Yunnan province and Muse city in Myanmar, snowball sampling method was used for collecting 147 Myanmar FSWs as our respondents. Cross - sectional survey by self designed questionnaire was conducted for demographic characteristics, Amphetamine -type stimulant （ATS） use, sexual behavior information. Meanwhile, HIV test and ATS test were conducted for all the respondents. Results 46 Myanmar cross -border FSWs were ATS positive. Methamphetamine and 3, 4- methylenedioxymethamphetamine （MDMA） were the two most commonly used drugs. 23 ATS -positive Myanmar cross - border FSWs did not insist in using condom after ATS use while having sex with male clients. ATS positive （OR = 3. 362,95% CI： 1. 625 -6. 956） and intercourse with male clients after ATS use （OR = 1.911,95% CI： 1. 007 -3. 625） were risk factors . Conclusion Some intervention targeting ATS use should be applied for Myanmar cross -border FSWs. In addition, the education and behavior intervention of condom use should continue to be strengthened.
