摘  要:目的分析3 076株尿液标本中分离细菌的分布及耐药性,为尿路感染的合理治疗提供依据。方法收集门诊及住院患者尿液标本并分离细菌,用Microscan-4细菌鉴定药敏仪进行鉴定和药敏实验,确证实验检测大肠埃希菌和肺炎克雷伯菌超广谱β-内酰胺酶。结果尿液标本中分离细菌株数位于前3位的依次是大肠埃希菌、屎肠球菌和肺炎克雷伯菌;女性尿液中大肠埃希菌所占比例显著高于男性(P<0.01);大肠埃希菌和肺炎克雷伯菌超广谱β-内酰胺酶的阳性率男性显著高于女性(P<0.01);大肠埃希菌和肺炎克雷伯菌的耐药率男性高于女性;屎肠球菌的耐药率男性和女性无差异。结论大肠埃希菌为尿路感染的首位病原菌,男性和女性尿液标本中分离细菌的菌种构成及耐药情况有显著差异。Objective To provide credible evidences for the treatment of urinary tract infection by studying the distribution and drug resistance of 3 076 bacteria strains isolated from urine specimens. Methods The bacteria strains identification and drug resistance isolated from urine specimens of inpatients and outpatients were tested by Microscan-4 system. The extended-spectrum beta-lactamases of E. coli and K. pneumoniae was detected by confirmatory test. Results The top three bacteria isolated from urine specimens were E. coli, K. pneumoniae and E. Faecium, respectively. The proportion of E. coli in the female urine specimens was significantly higher than that in the male urine specimens (P 〈 0.01). The positive rate of extended-spectrum beta- lactamases of E. coli and K. pneumoniae in the male patients was significantly higher than that in the female patients (P 〈 0.01); Drug resistance rate of E. coli and K. Pneumoniae isolated from the male patients was higher than that from the female's; Drug resistance rate ofE. Faecium isolated from male's and female's was no significant difference. Conclusion E. coli was the most predominant pathogen in urinary tract infec- tion. There exists a significant difference in urine bacteria stains composition and drug resistance Between the male and female patients.
