摘 要:目的分析使用改良型UG037370全自动热辐射刺激仪测量人体热痛阈值的影响因素,并计算其正常参考值。方法选择健康大学生496人(男188人,女308人)为受试者,使用改良型UG037370全自动热辐射刺激仪,以中指远端指腹为测量点。测试前先测量受试者指腹的基线回避反应温度(BET),再行热痛测试并记录回避反应潜伏期(WLT)和回避反应时的皮肤温度(EWT),顺序测量左右手,然后分析影响热痛测量的因素并计算其正常参考值。结果WLT与受试者测量部位BET值呈负相关(左手r=-0.462,P〈0.01;右手r=-0.411,P〈0.01);受试者左侧测痛点热痛阈值高于右侧(P〈0.01),男性与女性受试者左右测痛点热痛阈值差异无统计学意义(P〉0.05);通过计算得到左右两测痛点热痛阈值95%正常参考值(左手EWT37.0-47.2,WLT〈14.2;右手EWT36.4-46.6,WLT〈12.7)。结论热痛阈值测量受测量点局部表面温度及左右利手影响,但性别对其影响无统计学意义;本研究所得的正常参考值可为今后临床热痛测量提供一定参考。Objective To investigate and analyze the possible factors of thermal pain threshold mea-surement with modified UGO Basile Biological Apparatus in human volunteers. Methods Four hundreds and ninety-six healthy undergraduates( 188 male and 308 female) were received the modified UGO Basile Biological Apparatus for heat pain testing. The baseline epicutaneous temperature (BET) was measured at the right middle finger pulp, and then started to measure the thermal pain threshold. The withdrawal latency time (WLT) and epicutaneous withdrawal temperature(EWT) were recorded. One minute later the left middle finger pulp was also measured subsequently. Factors affecting heat pain were measured and the normal reference ranges of WLT and EWT were calculated. Results The results of linear regression analysis showed that the WLT and BET were negatively correlated(left r= -0.462, P〈 0.01; right r= -0.411, P〈 0.01 ). WLT and EWT at the left point were higher than that at right point (P 〈 0.01 ), and the gender differences of WLT and EWT were statistically insig- nificant( P 〉 0.05 ). The normal reference values of W LT (s) and EWT (℃)(left EWT 37.0-47.2, WLT 〈 14.2; right EWT36.4-46.6, WLT 〈 12.7) at the right point of 496 healthy undergraduates were obtained. Conclusion The thermal pain threshold measurement might he affected by the right handedness and gender has little influence for that. The normal reference ranges of the WLT and EWT at the specific points for volunteers could be served as a reference for thermal pain measurement.
