中西医结合治疗老年慢性阻塞性肺疾病急性发作期合并2型糖尿病临床观察

摘　　要：目的:观察血府逐瘀汤联合西药治疗老年慢性阻塞性肺疾病急性加重期(AECOPD)合并2型糖尿病的临床疗效。方法:将符合纳入标准的68例气虚血瘀型AECOPD合并2型糖尿病的老年患者随机分为两组各34例,对照组给予西医常规治疗,治疗组在对照组基础上加服血府逐瘀汤,疗程10天。结果:与治疗前比较,两组肺功能指标FEV_1、FEV_1/FVC均显著改善(P<0.05),治疗组治疗10天后FEV_1、FEV_1/FVC高于对照组(P<0.05);两组治疗10天后PaO_2、PaCO_2、SaO_2均较治疗前有所改善(P<0.05),治疗组治疗10天后上述指标改善显著优于对照组(P<0.05);两组治疗后10天血浆D-D均降低(P<0.05),APTT升高(P<0.05),治疗组上述指标明显优于对照组(P<0.05)。结论:血府逐瘀汤辅助治疗能够缓解AECOPD合并2型糖尿病患者血栓前高凝状态,改善肺功能及血气指标。Objective ： To observe the clinical effect of Xuefu zhuyu decoction combined with western medicine on elderly acute exacerbation of chronic obstructive pulmonary disease （AECOPD）with type 2 diabetes. Methods： 68 cases of elderly patients diagnosed with AECOPD and type 2 diabetes of qi deficiency and blood stasis syndrome were randomly divided into two groups, 34 cases in each group. The control group was treated only with conven- tional western medicine, while the treatment group were treated plus Xuefu zhuyu decoction on the basis of control group, 10 days as a course of treatment. Results ： Compared with before treatment, lung function indexes of both groups i.e. FEV1, FEV1/FVC were obviously improved （P 〈 0.05 ）; but after treatment forl0 days, FEV1, FEV1/FVC of treatment group were higher than those of control group （P 〈 0.05）. After treatment for10 days, PaO2, PaCO2, SaO2 of both groups were improved compared before treatment （ P 〈 0.05 ）, but the improvement of treatment group was significantly superior to control group （P 〈 0.05 ） ; and in both groups, D-D of plasma was lowered （P 〈 0.05 ）, APTr of plasma was increased （ P 〈 0.05 ）, but the above indexes of treatment group were significantly superior to control group （ P 〈 0.05 ）. Conclusion ： As an adjuvant therapy, Xuefu zhuyu decoction can relieve the prothrombotic hypercoagulative state of patients with AECOPD and type 2 diabetes, and improve the lung function and blood-qi indexes .
