超声在40岁以下女性乳腺癌诊断中的应用价值

摘　　要：目的探讨超声在诊断40岁以下女性乳腺癌中的应用价值。方法选取2014年1~12月越南一○三军人医院收治的71例乳腺占位性病变患者,行乳腺超声和乳腺钼靶检查,以病理结果为金标准,计算超声和乳腺钼靶对其的诊断敏感性、特异性及准确性。结果 71例患者中,40例乳腺癌,31例非乳腺癌。40例乳腺癌患者中,超声诊断32例,漏诊8例;乳腺钼靶诊断29例,漏诊11例;31例非乳腺癌患者中,超声诊断25例,误诊6例;乳腺钼靶诊断29例,误诊2例。超声诊断乳腺癌的准确性、敏感性、特异性为80.2%、80.0%、80.6%;乳腺钼靶诊断乳腺癌的准确性、敏感性、特异性为81.6%、72.5%、93.5%。结论超声在诊断40岁以下女性乳腺癌中有一定优势,当超声图像不能判定时,应行乳腺钼靶检查,以提高乳腺癌的诊断准确性。Objective To explore the value of ultrasound in diagnosis of breast cancer in women under 40 years old. Methods Seventy-one patients with breast lesions were selected from January 2014 to December in Vietnam 103 Military Hospital,and the breast lesions were examined by ultrasound and mammography with pathological results as the gold standard,then calculate the sensitivity, specificity and accuracy of ultrasound and mammography in diagnosis of breast cancer in women under the age of 40 years. Results In 71 cases,40 cases were breast cancer, another 31 cases were not breast cancer. In the 40 cases of breast cancer,32 cases were diagnosed by ultrasound and 8 cases were missed,29 cases were diagnosed by mammography and 11 cases were missed. In 31 cases of non breast cancer,25 cases were diagnosed by ultrasound and 6 cases were misdiagnosed,29 cases were diagnosed by mammography and 2 cases were misdiagnosed.The accuracy,sensitivity and specificity of ultrasound in the diagnosis of breast cancer were 80.2%, 80.0% ,80.6% ,respectively,and 81.6% ,72.5%,93.5% of mammography. Conclusion Ultrasound has a certain advantage in the diagnosis of breast cancer in women under 40 years old,however,when the ultrasound image cannot diagnose the cancer,mammography should be conducted in order to improve the diagnostic accuracy of breast cancer.
