高血压病常见中医证型量化诊断标准的探索性研究

摘　　要：目的:建立高血压病常见中医证型的量化诊断标准,探索中医证候诊断规范化研究的思路与方法。方法:以文献分析、专家咨询和临床观察为基础,建立高血压病常见中医证型诊断量表。从信度、效度、反应度等方面对各证型量表进行考评。采用综合权重法对条目进行赋权,建立高血压病常见中医证型宏观量化诊断模型、诊断阈值与程度分级标准。并对各证型标准进行临床诊断性试验。结果:编制的8个高血压病常见中医证型宏观诊断量表具有良好的信度、效度及反应度。在此基础上建立了高血压病8个中医证型量化诊断标准,确立了最佳证候诊断阈值以及证型分级诊断标准。临床诊断性试验结果显示,该标准具有较高的灵敏度、特异度、准确度及阳性似然比。结论:所构建的高血压常见中医证型宏观量化诊断标准具有良好的可行性、准确性和实用价值,为疾病证候规范化研究奠定基础。Objective： To establish quantification diagnostic standard of common traditional Chinese medicine（TCM） syndromes for hypertension and to provide ideas and methods for standardization diagnosis of TCM syndromes. Methods： Based on the literature survey, clinical epidemiological investigation and expert advice, common syndromes diagnosis scales of hypertension were established. The reliability, validity and responsiveness of these scales were investigated. Comprehensive weight method was applied to weight determining for items of scales in order to establish diagnosis models, diagnostic threshold and degree-grading standard. In addition, clinical diagnostic tests were executed for the diagnostic criteria of hypertension syndromes. Results： Eight hypertension common syndrome diagnosis scales were formed, and these scales showed good reliability, validity and responsiveness by comprehensive evaluation. On this basis, 8 diagnosis models were established, and the best diagnostic threshold and degree-grading standard were established. Clinical diagnostic test results showed that the quantification diagnostic standards had high sensitivity, specificity, accuracy and positive likelihood ratio. Conclusion： The established quantification diagnostic standard of common TCM syndromes for hypertension has favorable feasibility, accuracy, and practical value, which provides a foundation for standardization research of hypertension.
