血液透析患者社会应对方式与健康状况的相关性研究

摘　　要：目的探讨维持血液透析患者社会支持、应对方式对维持血液透析患者健康状况影响,为维持血液透析患者健康干预提供依据。方法采用整群抽样对临床189名维持血液透析患者运用社会支持量表、应对方式量表、健康状况量表进行调查。结果 185人中,有42名阳性筛选者,阳性检测率为22.70%;健康状况各维度得分在9个维度得分均高于中国常模,在躯体化、强迫、人际关系敏感、抑郁、焦虑、敌对、恐怖、偏执和精神病性维度组间差异具有统计学意义(P<0.05)。健康状况总分与社会支持各维度呈负相关(P<0.05),相关系数r分布在-0.415^-0.308;健康状况总分与应对方式积极维度呈负相关(P<0.05),与消极应对方式呈正相关(P<0.05),相关系数r分布在-0.218~0.274。多元回归显示:健康状况总分影响因素有年龄、婚姻状况、收支水平、支付方式、社会支持、应对方式。结论维持血液透析患者健康水平低于普通人群,应通过多种途径增加对患者的社会支持,增强患者应对能力,改善维持血液透析患者健康状况。Objective To discuss the effect of social support and coping styles of maintenance hemodialysis patients on their health status and provide basis for health intervention for maintenance hemodialysis patients. Methods 189 maintenance hemodialysis patients were selected with cluster sampling method. Social support scale, coping style scale and health scale were applied in the investigation. Results 42 out of 185 cases were positive. The positive detection rate was 22.70%. The health score of nine dimensions were higher than the norm in China. There were statistical differences between groups regarding somatization, force, interpersonal sensitivity, depression, anxiety, hostility, terror, paranoia and psychoticism dimension （P〈0.05）. Health status score was negatively correlated with social support in all dimensions （P 〈 0.05）. Health status score was negatively correlated with positive coping styles and positively correlated with negative coping styles （P 〈 0.05）. Multiple regression showed that the influencing factors of health status scores included age, marital status, income level, payment method, social support and coping styles. Conclusion As the health status of maintenance hcmodialysis patients is lower than general population, social support for these patients should be increased to improve their coping capacity and health status.
