射频消融房室结慢径路时不同类型交界性心律的特点及对复发与并发症的影响

摘　　要：目的观察房室结慢径路(PS)射频导管消融(RFCA)治疗房室结折返性心动过速(AVNRT)时不同类型房室交界性心律(JR)的特点,分析其复发率与并发症发生情况。方法选取我院2010年2月至2013年4月收治的经PS和RFCA治疗后AVNRT患者54例,观察不同频率、不同类型JR的特点,放电消融成功率及其与术后并发房室传导阻滞(AVB)发生率和复发率的关系。结果不同类型JR发生一过性AVB率和复发率间均存在一定差异,以持续型JR发生率最高,显著高于间歇型和递减型,P<0.05,有统计学意义。结论经PS和RFCA治疗后AVNRT患者间歇型和递减型JR发生一过性AVB和复发率较低,故中频JR和间歇型和递减型JR可作为提前预测AVB与复发的有效指标。Objective To investigate the features of different types of junctional rhythm （JR） in the treatment of atrioventricular node reentrant tachycardia （AVNRT） with slow atrioventricular nodal pathway （PS） and radiofrequency catheter ablation （RFCA）, as well as their influence on recurrence rate and the development of complications. Methods A total of 54 patients with AVNRT who were admitted to our hospital and treated with PS and RFCA from February 2010 to April 2013 were enrolled. The features of JR of different types and frequencies were observed, and the success rate of discharge ablation and its association with the incidence rate of postoperative atrioventricular block （AVB） and recurrence rate were analyzed. Results The incidence rate of transient AVB and recurrence rate showed significant differences between patients with different types of JR, and the patients with persistent JR had the highest incidence, rate, significantly higher than those with intermittent and decrescendo JR （P〈0.05）. Conclusion After treatment with PS and RFCA, the AVNRT patients with intermittent and decrescendo JR have a low incidence rate of transient AVB and a low recurrence rate, and therefore, intermediate-frequency JR and intermittent and decrescendo JR can be used as effective indicators for predicting AVB and recurrence.
