髋部骨折老年人中医体质分布及与骨密度关系

摘　　要：目的针对髋部骨折的老年人进行中医体质分布规律的调查,并且对老年髋部骨折与骨密度的关系进行分析和探讨,为预防老年髋部骨折制定干预措施提供帮助。方法入选2013年1月—2014年6月于我院进行治疗的髋部骨折老年患者124例作为研究观察对象,同期选取健康体检老年人124例作为对照组,针对两组患者进行中医体质分布规律的探查,并对两组患者骨密度进行测量和研究。结果健康体检老年人群中平和质最常见,占总体的47.58%,其次为气虚质,占总体的15.32%,差异具有显著性,P<0.05;在髋部骨折老年患者中气虚质、痰湿质、血瘀质、阴虚质和阳虚质占主导地位,组内比较差异具有显著性,P<0.05;健康体检老年人群平均骨密度明显高于髋部骨折老年患者,P<0.05;针对两组患者合并后,将骨密度和体质类型进行相关性的比较,差异具有统计学意义,P<0.01。结论在老年髋部骨折患者中气虚质、痰湿质、血瘀质、阴虚质和阳虚质为主要的体质影响因素,对比健康体检老年人群,髋部骨折老年患者骨密度显著下降,中医体质类型和骨密度有较为密切的相关性。Objective To investigate TCM constitution distribution law of senile patients with hip fracture, and to analyze and discuss the relationship between fracture and BMD, and offer help to prevent hip fracture of senile patients and make intervention measures. Method 124 senile patients with hip fracture were chosen as research objects, and at the same time 124 healthy senile patients were chosen as control group. TCM constitution distribution law of patients of the two groups was investigated, and their BMD was measured and studied. Result Mild constitution was most common among healthy senile patients, accounted for 47. 58%, secondly qi-deficiency constitution, accounted for 15. 32%, P 〈0. 05, qi-deficiency, phlegm-dampness, blood stasis, yin-deficiency and yang-deficiency constitutions were predominant among senile patients with hip fracture, and the difference was significant, P〈0. 05. The mean BMD of healthy senile patients was obviously higher than that of senile patients with hip fracture, P〈0. 05, combining with the 2 groups, the relevance between BMD and constitution were compared, and difference was statistically significant P 〈0. 01. Conclusion Qi-deficiency, phlegm-dampness, blood stasis, yin-deficiency and yang-deficiency constitutions were main influencing factors of senile patients with hip fracture. Compared with healthy senile patients, their BMD was obviously lower. TCM constitution type is closely related with BMD.
