摘　　要：【目的】探讨血清人附睾上皮分泌蛋白4（HE4）水平在子宫内膜病变中的表达及意义。【方法】采用ELISA方法检测子宫内膜癌患者（60例，A组）、子宫内膜非典型增生患者（10例，B组）和子宫内膜良性疾病患者（65例，C组）的血清HE4水平并比较。【结果】A组HE4水平为138．00pmol／L，显著高于B组42．10pmol／L和C组23．15pmol／L（P〈0．05）；血清HE4水平与子宫内膜癌患者年龄、是否绝经、病理类型、有无淋巴结转移、细胞冲洗液是否阳性无关（P〉0．05），而与临床FIGO分期、病理组织学分级及肌层浸润相关（P〈0．05）。【结论】HE4是子宫内膜癌较好的血清学肿瘤标记物，与子宫内膜癌的临床进展密切相关，对子宫内膜癌的早期发现和鉴别具有一定的临床诊断价值。[Objective]To study the expression and meaning of serum HE4 levels in endometrium diseased region. [Methods]The level of serum HE4 was measured by enzyme-linked immunosorbent （ELISA） in the serum specimens of 60 cases in the endometrial cancer group（ group A）, 10 cases in the atypical growth of en- dometrial group（group B） and 65 cases in endometrial benign diseases group（group C）. [Results]The serum levels of HE4 in group A were significantly higher than those in group B and group C [138.00 pmol/L vs 42.10 pmol/L and 23.15 pmol/L（ P %0.05）].The expressions of HE4 had no relationship with the age of patients , menostasia ,pathological types, migrator of lymph nodes and flushing fluid of cells （ P 〉0.05）, but related with clinical stages of FIGO, the grade of histopathology and tumor invasion （ P 〈0.05）. [Conclusion]HE4 may be a better serum marker which is closely relative with the clinical progressions and contributes to the ear ly detection and identification of endometrial cancer.
