择期剖宫产及阴式分娩对新生儿Apgar评分及NBNA评分的影响

摘　　要：目的探讨择期剖宫产及阴式分娩对新生儿Apgar评分及NBNA评分的影响。方法回顾性分析2012年2月~2014年8月在我院住院分娩的358例足月新生儿的临床资料,其中择期剖宫产的179例作为择期剖宫产组;另外经阴式分娩的179例作为阴式分娩组;分析比较两组新生儿出生第1、5分钟的Apgar评分、出生第2~3天的NBNA评分、出生1 d内免疫指标的差异性。结果择期剖宫产组与阴式分娩组新生儿出生第1、5分钟的Apgar评分比较,差异无统计学意义（P〉0.05）;择期剖宫产组与阴式分娩组出生第2~3天的NBNA评分比较,差异无统计学意义（P〉0.05）;两组新生儿出生1 d内的免疫指标比较,阴式分娩组CRP、Ig A水平低于择期剖宫产组,Ig G、Ig M水平高于择期剖宫产组;两组比较,差异有统计学意义（P〈0.05）。结论择期剖宫产与阴式分娩的足月产新生儿近期健康状况及神经行为均无显著性差异,但阴式分娩可能改善新生儿的免疫功能,而择期剖宫产可能降低新生儿的免疫功能,增大预后不良的风险。Objective To explore the influence of selective caesarean section and vaginal delivery on neonate′s Apgar and NBNA scores. Methods From February 2012 to August 2014,clinical data from 358 full-term neonates delivered in our hospital were retrospectively analyzed.Among them,there were 179 cases by selective caesarean section and were classified into selective caesarean section group.The rest by vaginal delivery was categorized into another group.The1 min and 5 min-Apgar scores after giving birth and NBNA score 2ndto 3rdday during birth, and differences in immune indexes within one day after delivery in neonates were compared and analyzed between two groups. Results There was no statistical difference in Apgar score 1 min and 5 min after giving birth between the two groups（P〉0.05）.NBNA score in the two groups was not displayed statistical difference from 2ndto 3rdday after delivery（P〉0.05）.In comparison with immune indexes of neonates within one day after birth,C-reactive protein and Ig A level in the vaginal delivery group was lower than that in the selective caesarean section group respectively,and levels of Ig G and Ig M in the vaginal delivery group were higher,which displayed statistical differences in immune indexes levels（P〈0.05）. Conclusion There is no remarkable difference in short-term health condition and neurobehavior in full-term neonates by selective caesarean section and vaginal delivery,but the latter can improve neonatal immune functions,while selective caesarean section may lower immune response and increase the risk of poor prognosis.
