2011年东营地区鲍曼不动杆菌耐药基因同源性研究

摘  要:目的了解东营地区临床分离鲍曼不动杆菌的耐药性以及耐药基因同源性。方法收集2011年1-12月东营地区三级综合医院各类临床标本中分离的251株鲍曼不动杆菌,用K-B法进行药敏试验,PCR扩增主要碳青霉烯酶基因,用ERIC-PCR法进行DNA同源性分析。结果 251株鲍曼不动杆菌对哌拉西林耐药率最高,为76.2%,其次为甲氧苄啶/磺胺甲恶唑72.1%、头孢噻肟70.1%;耐亚胺培南/美罗培南鲍曼不动杆菌23株。同时携带OXA-23gp、OXA-24gp、OXA-64gp基因20株,携带OXA-58gp、OXA-64gp和PER基因3株;同源性分析显示,A1型15株,A2型1株,B型3株,C型2株,D型2株。结论东营地区鲍曼不动杆菌耐药严重,尚未发现同一克隆株的流行播散。Objective To know the drug resistance and resistance gene homology of clinical isolated Acinetobacter bauman- nil in Dongying. Methods Totally 251 strains of A. baumannii isolated from infectious clinical specimens from Jan to Dec 2011 were collected in three grade hospital of Dongying. The drug resistance tests were performed using K - B method. The resistant mechanism to carbapenem of A. baumannii were assayed by PCR. The homological analysis was performed by means of the Enterobacterial Repetitive Intergenic Consensus - Polymerase Chain Reaction ( ERIC - PCR). Results The drug resistance rate of the 251 strains of A. baumannii to piperacillin was the highest (76.2%), followed by the trime- thoprim/sulfamethoxazole (72.1% ) and cefotaxime ( 70. 1% ). Totally 23 strains of imipenem resistant and/or meropenem A. baumannii,20 strains carrying both of OXA -23 gp, OXA -24gp and OXA -64gp gene, 3 strains carrying both of OXA - 58gp, OXA- 64gp and PER genes. Homology analysis showed, 15 strains were type. A1,1 were type A2,3 were type B,2 were type C,2 strain were type D. Conclusion The drug resistance of A. baumannii in Dongying area is high. And spread of the same clone of the popular has not found yet.
