心率震荡与慢性心力衰竭患者左心室收缩功能的相关性研究

摘　　要：目的：研究心率震荡（heart rate turbulence，HRT）与慢性心力衰竭（chronic heart failure，CHF）患者左心室收缩功能的相关性及其意义。方法采用分层随机化分组方法选择80例 CHF 患者作为观察组，70例非 CHF 心脏病患者作为对照组，均评估心功能及检查动态心电图（Holter）、心脏超声。观察指标：HRT参数：震荡初始（TO）与震荡斜率（TS）；左室收缩功能指标：左室射血分数（LVEF）及左室舒张末期内径（LVEDD）。结果CHF 患者 TS 与 LVEF 呈正相关（r ＝0．881，P ＜0．01），与 LVEDD 呈负相关（r ＝－0．655， P ＜0．01）；TO 与 LVEF 呈负相关（r ＝－0.906，P ＜0．01），与 LVEDD 呈正相关（r ＝0．682，P ＜0．01）。CHF 患者 LVEDD 越大，LVEF 越低，则 TS 越低，TO 越高，心衰程度越重。结论HRT 与左室收缩功能具有显著相关性，可用于评估 CHF 病情严重程度及预测预后。Objective To study the correlation of heart rate turbulence（HRT）and left ventricular systolic function in patients with chronic heart failure（CHF）and its significance.Methods According to random principle, 80 patients with CHF and 70 cases without heart failure with heart disease were enrolled and classified into two groups：research group（n =80）and control group（n =70）.The two groups of patients were accepted the cardiac function evaluation and ambulatory electrocardiogram,the Doppler echocardiography.The observation indexes：（1 ） HRT parameters：turbulence onset（TO）and turbulence slope（TS）;（2）left ventricular systolic function indexes：left ventricular ejection fraction（LVEF）,left ventricular end -diastolic diameter（LVEDD）,etc.Results The TS was positively related to LVEF（r =0.881,P 〈0.01）and negatively related to LVEDD（r =-0.655,P 〈0.01）,the TO was negatively related to LVEF（r =-0.906,P 〈0.01）and positively related to LVEDD（r =0.682,P 〈0.01）in CHF group.In CHF group,the greater the LVEDD,the LVEF was lower,so the lower the TS,the higher the TO,the more severe heart failure.Conclusion There was a significant correlation between HRT and left ventricular systolic function.The HRT can be used for evaluating illness severity and predicting prognosis in patients with CHF.
