改进急诊抢救措施在老年高血压危象患者中的应用效果

摘　　要：目的研讨老年高血压危象患者采用改进急诊抢救方案治疗的临床价值。方法自本院2013年12月至2015年1月收集的老年高血压危象病例中选取86例并随机分组，对照组43例按常规急诊科标准抢救，观察组43例制定改进急诊抢救方案积极治疗，观察两组的总体抢救效果，并对两组抢救前后的收缩压（SBP）、舒张压（DBP）和心率（HR）变化情况进行比较。结果①观察组的总抢救率为93．O％，相比对照组的72．1％明显提高，两组比较差异有统计学意义（P〈0．05）。②抢救前，两组的SBP值、DBP值和HR值相比基本相当，两组比较差异无统计学意义（P〉0．05）。抢救后，除HR值外，观察组两项血压指标值相比对照组均明显降低，两组比较差异有统计学意义（P〈0．05）。结论对老年高血压危象患者实施改进后的急诊抢救方案抢救，重点在于快速平稳降压，并给予抗惊厥、吸氧和对症支持等措施处理，以显著提高抢救效果，促进患者恢复。Objective To discuss clinical value of improved emergency rescue measures in elderly patients with hypertensive crisis. Methods 86 elderly patients with hypertensive crisis in our hospital from December 2013 to January 2015 were selected and divided into two groups. 43 patients in control group underwent emergency department conventional standard rescue, while 43 patients in observation group underwent active improved emergency rescue measures. Observed the overall rescue effect, and compared systolic blood pressure （SBP）, diastolic blood pressure （DBP） and heart rate （HR） before and after rescue of two groups. Results （~） The overall rescue effective rate in observation group was 93.0%, significantly higher than 72.1% in control group, with statistically significant difference （P〈0.05）. （~ There were no statistically significant differences in SBP, DBP, and HR between two groups before rescue. After rescue, in addition to HR, SBP and DBP of observation group were significantly lower than those of control group, with statistically significant differences （P〈0.05）. Conclusions Improved emergency rescue measures in elderly patients with hypertensive crisis lays emphasis up on fast and smooth decompression, and anti-convulsant, oxygen-uptake, symptomatic and supportive measures, to significantly improve the rescue effect and promote the recovery of patients.
