辛开苦降法治疗溃疡性结肠炎临床研究

摘　　要：目的:观察辛开苦降法治疗溃疡性结肠炎的临床疗效。方法:将68例患者随机分为治疗组和对照组,每组33例。对照组服用柳氮磺胺嘧啶治疗;治疗组在对照组基础上加用辛开苦降法治疗,具体用药为黄连10 g,黄芩10 g,党参12 g,清半夏9 g,干姜8 g,炙甘草8 g,陈皮12 g,炒白术12 g,白芍15 g,防风12 g,白头翁15 g,桑白皮12 g,大枣6个。若患者便血较多加用生地炭、侧柏炭、地榆炭等药物收敛止血;若患者便脓较多则加用败酱草、金银花、秦皮等清热解毒祛湿;若腹痛明显可加用乌梅、细辛、蜀椒等药温脏驱寒止痛;若胃脘胀闷、纳差、乏力明显者则加用黄芪、党参、厚朴、焦三仙,益气健脾、祛湿止泻;若肾虚征象明显者则加用温肾收敛之品。每日1剂。共治疗12周,结束后判定临床疗效和复发率。结果:治疗组35例,痊愈6例,显效26例,无效3例,有效率为91.4%;对照组33例,痊愈3例,有效20例,无效10例,有效率为69.7%。两组临床疗效比较,差异有统计学意义(P<0.05)。结论:辛开苦降法治疗溃疡性结肠炎显著,且复发率低。Objective：To observe the clinical effect of the treatment of ulcerative colitis with the method of Xinkai Kujiang. Methods：68 cases were randomly divided into the treatment group and the control group,33 cases in each group. The control group was given salazosulfadimidine,and the treatment group was treated with Xinkai Kujiang method based on the control group. Drug component：Huanglian（ Coptis chinensis） 10 g,Huangqin（ Radix Scutellariae）10 g,Dangshen（ Codonopsis） 12 g,Qingbanxia（ Rhizoma Pinelliae） 9 g,Ganjiang（ Rhizoma Zingiberis） 8 g,Zhigancao（ Baked Licorice） 8 g,Chenpi（ Dried Tangerine Peel）12 g,Chaobaizhu（ White Atractylodes Rhizome） 12 g,Baishao（ Radix Paeoniae Alba） 15 g,Fangfeng（ Radix Sileris） 12 g,Baitouweng（Pulsatilla） 15 g,Sangbaipi（White Mulberry Root Bark） 12 g,Dazao（Jujube） 6 pieces. If patients with more blood in the stool with Shengditan（ Rehmanniae Carbon）,Cebaiye（ Cacumen Platycladi）,Diyutan（ Sanguisorba Carbon） drugs for astringency and hemostasis;If patients with pus more plus Baijiangcao（ Herba Patriniae）,Jinyinhua（ Honeysuckle）,Qinpi（ Cortex Fraxini） for clearing heat,detoxifying and removing dampness; If obvious abdominal pain can be treated with Wumei（ Ebony）,Xixin（ Asarum）,Shujiao（ Sichuan Pepper） and other drug warming viscera,driving cold and relieving pain;If stomach up stuffy,loss of appetite,fatigue significantly with Huangqi（ Astragalus）,Dangshen（ Codonopsis pilosula）,Houpu（ Magnoliae officinalis）,Jiaoshanzha（Charred Fructus Crataegi）,Jiaoshenqu（ Medicated Leaven）,Jiaomaiya（ Coking Malt） for replenishing Qi to invigorate the spleen,clearing damp and antidiarrheal;If obviously kidney deficiency signs,addition warming the kidney and convergence drugs,daily 1 agent. A total of 12 weeks of treatment,after the end of the treatment,determined curative effect and the recurrence rate.Results：35 cases in the treatment group,6 cases were recovered,26 cases were markedly ef
