新生儿乙型肝炎疫苗初免及加强免疫应答与出生体重的相关性观察

摘　　要：目的观察新生儿乙型肝炎(乙肝)疫苗初免及加强免疫应答与出生体重的关系。方法使用寿光市新生儿乙肝疫苗免疫持久性随访研究资料,选择2009年出生,完成了新生儿乙肝疫苗全程免疫和免后抗体检测,足月出生,出生体重记录清楚的儿童为观察对象。对乙肝疫苗初免和加强免疫检测结果进行统计分析。结果 968名新生儿乙肝疫苗全程免疫后抗-HBs保护应答率随出生体重增加而增高,差异有统计学意义(χ2=16.847,P=0.005);抗-HBs几何平均滴度(GMT)与出生体重存在正相关关系(rs=0.083,P=0.010)。免疫后5年,对抗-HBs<10 m IU/ml的儿童进行1剂次乙肝疫苗加强免疫,加强免疫后抗体水平与出生体重无相关性(rs=0.011,P=0.875)。结论出生体重是新生儿乙肝疫苗免疫应答的影响因素,出生体重与初免应答存在正相关关系,而与加强免疫则无相关性。Objective To investigate the relationship between immune response to primary and booster immunization of hepatitis B vaccine and the birth weight in neonates. Methods The data were based on a long-term follow-up study on immune persistence of hepatitis B vaccine in neonates in Shouguang City. The infants born at full term in 2009 with accurate birth weight records, full dose of hepatitis B vaccine and antibody tests were recruited. The immune response to primary and booster immunization of hepatitis B vaccine was analyzed. Results A total of 968 newborns were examined. Their antibody response rate rose significantly as their weight gained （χ^2 = 16. 847, P = 0.005 ） .There existed positive correlation between geometric mean titres of anti-HBS and birth weight（rs = 0. 083, P = 0.010）. Five years after primary immunization, the infants with anti-HBs less than 10mIU/ml were vaccinated with booster hepatitis B vaccines. However, the antibody levels after booster immunization had no obvious correlation with the birth weight （ rs = 0. 011, P = 0. 875 ）. Conclusions Birth weight is an influencing factor of the response to hepatitis B vaccine in newborns. Birth weight is positively correlated with primary immunization response to hepatitis B vaccine while not correlated with response to booster immunization.
