摘 要:目的运用320层CT双入口灌注成像技术,观察复治涂阳肺结核患者病灶的灌注特点,研究病灶血液循环与结核病复治的关系。方法采用回顾性调查方法,对在解放军第三。九医院确诊的54例肺结核患者进行研究,其中结核球6例,浸润性病灶21例,空洞型病灶8例,多形性病灶19例,根据治疗效果,分为初治治愈组30例、复治组24例(首次复治10例、多次复治14例)。对初治治愈患者的灌注值与复治涂阳患者及初次复治与多次复治者的病灶灌注参数进行统计分析。结果初治治愈组患者的肺动脉血流量(pulmonary flow,PF)值为(46.4±9.2)ml·min^-1·(100m1),高于复治组[(25.9±7.6)ml·min^-1·(100ml)^-1],差异有统计学意义(t=8.70,P=0.001);而其支气管动脉血流量(bronchial flow,BF)值为(18.9±10.0)ml·min^-1·(100m1),低于初治组[(24.8±8.8)ml·min^-1·(100m1)^-1],差异有统计学意义(t=2.30,P=0.025)。多次复治组PF值为(20.7±4.6)ml·min^-1·(100ml),低于首次复治组E(32.6±5.3)ml·min。·(100m1)^-1],差异有统计学意义(t=5.88,P=0.001),而其BF值为(25.8±3.2)ml·min^-1·(100m1),高于首次复治组[(22.3±5.2)ml·min^-1·(100m1)^-1],但差异无统计学意义(t=6.41,P=0.648)。结论复治涂阳肺结核患者病灶的肺动脉血供明显低于初治治愈病灶,推测局部血液循环差是结核病患者复治的原因之一。Objective To observe the perfusion characteristics using the dual input perfusion with 320 detector CT and research the relationship between lesion blood circulation and tuberculosis retreatment. Methods Fifty-four cases with pulmonary tuberculosis including tuberculoma in 8 cases, infikrative lesion in 21 cases, cavitary lesion in 8 cases and mixed lesion in 19 cases were enrolled retrospectively in the 309th Hospital of Chinese People's Liberation Army. All patients were divided into initial treatment and cure group (30 cases) and retreatment group (24 cases) including first retreatment in 10 cases and multiple retreatment in 14 cases according to the efficacy. The perfusion parameters were analyzed statistically. Results The pulmonary flow (PF) values of initial treatment and cured group (46.4±9.2) ml·min^-1 . (100 ml) i was higher than that of retreatment group (25.9±7.6) ml·min^-1 . (100 ml)^-1(t= 8. 70, P=0. 001). The bronchial flow (BF) values of initial treatment and cured group (24.8±8.8) ml ·min-1·(100 ml)^-1 was higher than that of retreatment (18.9±10.0) ml·min^-1(100 ml) 1 (t=2.30,P=0. 025). The PF value of multiple retreatment group (20.7±4.6) ml·min^-1 . (100 ml)^-1 was lower than that o{ first retreatment group (32.6±5.3) ml·min^-1 o (100 ml) 1 (t=5.88,P=0. 001). The BF value of multiple retreatment group (25.8±3.2) ml·min^-1 , (100 ml)^-1 was higher than that of first retreatment group (22.3±5.2) ml. min^-1·(100 ml)^-1, but the difference was not significant statistically (t=6.41,P=0. 648). Conclusion The pulmonary blood supply in the lesion of patients with retreatment smear-positlve pulmonary tuber culosis is significant lower than that in patients with initial treatment and cured tuberculosis. It is speculated that the local blood circulation worse is one of the reasons for the relapse of tuberculosis.
