弹性成像技术对甲状腺癌颈部淋巴结转移诊断价值

摘　　要：目的探讨超声弹性成像技术对甲状腺癌病人颈部淋巴结转移诊断价值。方法对62例甲状腺癌病人的169枚颈部淋巴结进行二维超声、彩色多普勒超声及弹性成像检查,其中弹性成像采用弹性图评分,以手术或穿刺活检后病理结果为标准,比较二维超声、彩色多普勒超声与弹性成像对甲状腺癌颈部淋巴结转移的诊断价值。结果二维超声、彩色多普勒超声及弹性成像诊断甲状腺癌病人颈部淋巴结转移的灵敏度分别为77.1%、83.3%、92.7%,特异度分别为74.0%、76.7%、80.8%,准确度分别为75.5%、80.4%、87.6%。弹性成像诊断甲状腺癌病人颈部转移淋巴结的灵敏度、特异度和准确度明显高于二维及彩色多普勒超声,差异有显著性（χ2=11.05~17.00,P〈0.01）。结论超声弹性成像对甲状腺癌病人颈部淋巴结转移诊断价值高于二维及彩色多普勒超声。Objective To assess the value of sonoelastography （SEG） in the diagnosis in thyroid cancer patients with cer- vical lymph node （CLN） metastasis. Methods Two-dimensional ultrasonography （2DU）, color Doppler ultrasonography （CDU） and SEG were carried out for 169 CLNs in 62 patients with thyroid cancer. Of which, the images of SEG were scored using elastography. Taking operative or needle biopsy pathology as the standard, the diagnostic value between 2DU, CDU and SEG for metastatic CLNs was compared. Results The sensitivity of 2DU, CDU and SEG in the diagnosis of CLN metastasis in patients with thyroid cancer was 77.1%, 83.3%and 92.7%, respectively, the specificity of the three methods was 74.0%, 76.7% and 80.8%, and the accuracy was 75.5%, 80.4% and 87.6% The sensitivity, specificity and accuracy of SEG were obviously higher than that of 2DU and CDU, the difference was significant （X2 =11.05--17.00,P〈0.01）. Conclusion The value of sonoelastog- raphy in the diagnosis of cervical lymph node metastasis in patients with thyroid cancer is higher than two-dimensional ultrasonog- raphy and color Doppler ultrasonography.
