摘 要:目的探讨体外构建的组织工程骨软骨复合体修复羊膝关节骨软骨复合缺损的效果。方法在自行设计制造的双腔搅拌式生物反应器内,对复合于β-磷酸三钙(β-TCP)支架材料上的羊骨髓间充质干细胞(MSCs)同时进行成骨和成软骨诱导,构建骨软骨复合体。模仿马赛克骨软骨移植术用构建的复合体修复实验组羊膝关节骨软骨复合缺损,对照组使用单纯β-TCP支架修复缺损。术后12周对修复关节进行大体、X线和Ⅱ型胶原免疫组化检查。结果 X线检查显示实验组植入物降解快速,与周围骨质融合好;大体观察见实验组修复软骨颜色和质地与周围正常软骨相似,关节面平整,而对照组修复区可见明显凹陷,有部分软骨样组织;Ⅱ型胶原免疫组化检查显示实验组修复区软骨染色强,胶原纤维整齐。结论利用自行设计制造的双腔搅拌式生物反应器体外构建的骨软骨复合体可有效修复羊膝关节骨软骨复合缺损。Objective To assess the effect of in vitro constructed tissue-engineered osteochondral complex on repairing articular osteochondral defect in goats. Methods Using self-designed and manufactured double-chamber stirring bioreactor, marrow-derived mesenchymal stem cells (MSCs) of goat that compounded in β-tricalcium phosphate (β-TCP) scaffolds were induced for formation of bone and cartilage to construct a bone-cartilage complex. Imitating mosaic bone and cartilage transplantation, the created complex was used to restore the combined osteochondral defect of knee joints in goat models, and those in the control group received only β-tricalcium phosphate (β-TCP) scaffolds. After 12 weeks of transplantation, a general inspection, X-ray and Ⅱ col lagen immunohistochemical staining were carried out for the repaired joints. Results An X-ray examination showed rapid degra- dation of the implants was well fused with surrounding bone in the experimental group, and general observation indicated that the color and texture restored were similar to surrounding cartilage, the joint surface was smooth. In the control group, a pitting could be seen at repaired area with fraction of chondroid tissue. Immunohistoehemieal staining of type Ⅱ collagen in the experimental group demonstrated that in the repaired area, cartilage staining was strong and collagen fibers arranged regularly. Conclusion Applying self-designed and manufactured double-chamber stirring bioreactor to make in vitro tissue-engineered osteochondraI composite grafts can effectively restore defect of bone and cartilage of knee joint in goats.
