InterTAN与逆行髓内钉结合空心钉治疗股骨干合并同侧股骨颈骨折的疗效比较

摘　　要：目的：探讨InterTAN与逆行髓内钉（GSH）结合空心钉两种治疗方法在股骨干合并同侧股骨颈骨折中的应用及临床疗效。方法：回顾分析从2009年10月至2012年12月本中心收治的股骨干合并同侧股骨颈骨折患者26例,采用InterTAN系统治疗12例（A组）,逆行髓内钉结合空心钉治疗14例（B组）。术后1,3,6,12个月及每年随访1次,通过临床影像学及Friedman-Wyman评分标准进行骨折愈合情况及疗效评定。结果：全部病例均获得随访,随访时间12-38个月,平均为25.6个月。两组患者在术中出血量、手术时间、术后引流量、股骨干愈合时间、股骨颈愈合时间及Friedman-Wyman功能评分方面两组患者差异均无统计学意义（P〉0.05）。结论：股骨干复合同侧股骨颈骨折临床治疗应采用个体化治疗原则,InterTAN适用于股骨颈骨折合并股骨近端骨折患者,GSH结合空心钉适用于股骨颈骨折合并股骨干及远端骨折患者,两种治疗方式临床效果相当。Objective：To discuss application and efficacy comparison of InterTAN and GSH combined with hollow nail in treatment of ipsilateral fractures of femoral neck and shaft.Methods：Retrospective analysis was performed on 26 cases of ipsilateral fractures of femoral neck and shaft treated in our hospital from October 2009 to December 2012,12 cases in group A were treated with InterTAN system and 14 cases in group B were treated with GSH combined with hollow nail.After 1month,3months,6months,12 months and every year,the patients were followed up.The fracture healing and the curative effect was evaluated by clinical imaging and Friedman-Wyman scoring criteria.Results：All patients were followed up for 12~38months,with an average of 25.6 months.There was no significant difference between the two groups in terms of blood loss,operative time,postoperative drainage volume,healing time of femoral shaft,healing time of femoral neck and Friedman-Wyman score（P0.05）.Conclusion：The treatment of ipsilateral fractures of femoral neck and shaft should be individualized.InterTAN is suitable for femoral neck fracture combined with proximal femoral fracture,and GSH combined with hollow nail is suitable for femoral neck fractures with femoral shaft and distal fractures.The clinical effect of the two methods is similar.
