血清钙、心肌酶谱、脑钠素和心脏彩超指标在肺炎心衰患儿中的变化分析

摘　　要：目的:了解肺炎合并心力衰竭患儿血清钙、心肌酶谱、脑钠素和心脏彩超指标变化情况。方法:选取我院2012年4月至2015年4月收治的肺炎伴心力衰竭婴幼儿80例(心衰组)、同阶段普通肺炎婴幼儿80例(普通肺炎组)和同期体检正常婴幼儿80例(对照组)。对比各组心肌酶谱、血清钙、磷、镁、脑钠素和心脏彩超指标。结果:治疗前血钙、镁心衰组均显著低于普通肺炎组(P<0.05),血清脑钠素显著高于普通肺炎组,左室射血分数、左室短轴缩短率、主动脉血流峰值流速和肺动脉血流峰值流速心衰组显著低于普通肺炎组(P<0.05)。治疗前和治疗7 d后肌酸激酶、肌酸激酸同工酶、肌红蛋白、肌钙蛋白、天冬氨酸氨基转移酶和乳酸脱氢酶心衰组均显著高于普通肺炎组(P<0.05)。结论:心肌酶谱及血清钙可作为诊断肺炎合并心力衰竭患儿心肌损伤的重要指标,脑钠素和心脏彩超可用于评价断肺炎合并心力衰竭患儿心功能的指标。Objective： To analyze serum calcium, myocardial enzyme, brain natriuretic peptide and cardiac ultrasound index changes in children with pneumonia complicated with heart failure. Methods： Eighty cases of pneumonia complicated with heart failure（HF） treated in our department from April 2012 to April 2015 were selected as HF group. At the same period 80 cases of pneumonia and 80 cases of normal infants were respectively served as normal group and control group. The myocardial enzymes, serum calcium, phosphorus, magnesium, brain natriuretic peptide and cardiac ultrasound index were compared in each group. Results： The levels of CK, CK-MB, Mb, TNT, AST and LDH before treatment and 7 days after treatment in the HF group were significantly higher than those in the normal group （ P 〈 0. 05 ）. The calcium and magnesium before treatment in the HF group were significantly lower than those in the normal group （ P 〈 0.05 ）. BNP prior to treatment in the HF group wassignificantly higher than that in the normal group. LVEF, LVFS, PEVP and PFVA in the HF group were significantly lower than those in the normal group （ P 〈 0.05 ）. Conclusion ： Myocardial enzymes and serum calcium can be used as an important indicator in the diagnosis of myocardial injury in children with pneumonia complicated with heart failure. BNP and cardiac ultrasound may be used to evaluate indicators of pneumonia and heart function.
