摘 要:目的需氧性阴道炎(aerobic vaginitis,AV)患者阴道内菌群失调是其致病的主要原因,分析易于培养的肠杆菌中产β-内酰胺酶情况,为临床合理使用抗生素提供依据。方法应用三维抑制试验回顾性分析2013至2014年713例本院确诊为需氧性阴道炎患者送检的分泌物经中的肠杆菌产头孢菌素酶(Amp C)、超广谱β-内酰胺酶(ESBL)、和超超广谱β-内酰胺酶(SSBL)的情况。结果各种肠杆菌β-内酰胺酶表型中,大肠埃希菌、阴沟肠杆菌、产气肠杆菌产ESBL≥26%属于高产类型;阴沟肠杆菌产Amp C酶≥26%属于高产类型,且所有菌株产β-内酰胺酶均≥10%,提示在需氧性阴道炎患者体内肠杆菌β-内酰胺酶表型比例较高。结论针对需氧性阴道炎合理使用β-内酰胺类抗生素避免耐药株产生显得非常重要;并且需氧性阴道炎患者的β-内酰胺酶阳性菌株可能是继发其他感染的原发菌原群灶。Objective Vaginal flora imbalance is the main cause of patients with aerobic vaginitis( AV). Analysis the situation of the β-lactamase production of easily cultured enterobacter can provide the basis for proper use of antibiotics in clinic.Methods Three-dimensional inhibition test was used to retrospectively analyze 713 cases diagnosed with AV in our hospital from2013 to 2014,the situation of enterobacter producing Amp C,ESBL,and SSBL was tested. Results In all phenotypes of enterobacter β-lactamase,ESBL production of escherichia coli,enterobacter cloacae and enterobacter aerogenes is greater than or equal to 26% and they belong to the high yield type; Amp C production of enterobacter cloacae is equal to or more than 26% and it belong to the high yield type,and β-lactamase production of all strains is greater than or equal to 10%,which prompted β-lactamase phenotype had higher proportion in aerobic vaginitis patients. Conclusion It is very important to β-lactam antibiotics properly in the aerobic vaginitis to avoid resistant strains,and β-lactamase aerobic positive strains in vaginitis patients may be the primary bacterial foci of secondary infection.
