电针配合威灵仙关节腔内注射对大鼠膝骨关节炎MMPs及HA的影响

摘　　要：目的探讨电针配合威灵仙膝关节腔内注射对大鼠膝骨关节炎基质金属蛋白酶(MMPs)及关节液透明质酸(HA)影响的作用机制。方法将40只大鼠随机分为对照组、电针组、关节腔注射组、电针+关节腔注射组,采用膝关节腔内注射弗氏剂的方法建立膝骨关节炎动物模型;电针组取三阴交、足三里、行间、血海、太冲、阳陵泉及主穴以下旁开0.5 cm为配穴电针刺激20 min,关节腔注射组于关节腔内注射威灵仙注射液;电针+关节腔注射组治疗方法为电针+关节腔注射;观察膝关节肿胀率及消肿时间,采用酶免疫吸附试验测定血清MMPs(MMP-1、MMP-2、MMP-3、MMP-9、MMP-13)阳性表达水平,关节腔积液中白介素(IL-1β)、HA含量。结果与电针组及关节腔注射组比较,电针+关节腔注射组膝关节肿胀率、消肿时间、MMP-1、MMP-9表达水平明显降低,HA明显提高(P<0.05)。结论电针配合威灵仙膝关节腔内注射治疗大鼠膝骨关节炎的作用机制可能与抑制MMPs过度表达并增加HA分泌而促进局部炎症吸收,延缓关节软骨退行性变相关。Objective： To investigate the effect and mechanism of electro acupuncture combined with Weilingxian intra-articular injection on MMPs and HA in rats with knee osteoarthritis. Methods： 40 rats were randomly divided into the control group,electroacupuncture group,Weilingxian group,electroacupuncture plus acupoint injection group. Osteoarthritis animal model was established with the method of intra articular injection of the knee joint. Electroacupuncture group took electroacupuncture below 0.5 cm of Zusanli,Sanyinjiao and Xingjian,Xuehai,Taichong,Yang Ling Quan and the main points for 20 minutes. Weilingxian group took intra-articular injection of Weilingxian. Electroacupuncture plus acupoint injection group received both treatment above. The knee swelling rate and joint swelling time were observed,and the following data was determined by enzyme-linked immunosorbent assay： positive expression level of serum MMPs（MMP-1,MMP-2,MMP-3,MMP-9,MMP-13）,IL-1β in articular effusion and the content of HA. Results： Knee joint swelling rate,swelling time of MMP-1,MMP-9 expression levels in electroacupuncture plus point injection group were significantly lower than those of Weilingxian group,and HA was obviously improved（P 0.05）. Conclusion： The mechanism of electroacupuncture combined with knee intra-articular injection of Weilingxian on knee osteoarthritis in rats should be related to the inhibition of overexpression of MMPs and the increase the secretion of HA,so as to promote inflammation absorption and delay cartilage degenerative change.
