创伤性肩关节前不稳定康复治疗与手术治疗效果评价

摘　　要：目的：分析探讨创伤性肩关节前不稳定康复治疗与手术治疗的临床效果对比。方法：选择我院2013年1月～2013年12月之间收治的68例创伤性肩关节前不稳定患者作为观察对象，根据患者的治疗方式将其分为康复治疗组和手术治疗组分别52例和16例。康复治疗组采取制动、肌力和保护性康复训练；手术治疗组行关节盂唇、前关节修复成形术，分析2组的结果差异。结果：2组的治疗优良率差异小，无统计学意义（P〉0．05）。康复治疗组患者随访12个月，存在肩部不适的有5例、后伸受限的有3例、前屈受限有1例；手术治疗组中有2例后伸活动受限，在剧烈活动下出现后肩前部不适的患者有2例，其他临床症状消失。结论：创伤性肩关节前不稳定患者的康复治疗具有很好的效果，但是需要较长的时间恢复，且会受到患者治疗依从性、前关节囊损伤情况等因素的影响，治疗失败的患者需行手术治疗，以促进关节功能的恢复。Objective： To analyze before traumatic shoulder joint instability rehabilitation treatment compared with the clinical effect of surgical treatment. Methods： Our hospital between January 2013 - December 2013 treated 68 cases of patients with traumatic shoulder joint instability before as research object, according to patients treatment, it can be divided into rehabilitation group and surgery group 52 cases and 16 cases respectively. The treatment group take braking, muscle strength and protective rehabilitation training; Before surgery group of glenoid cavity lip, joint repair plasty. Analysis differences between the results of two groups. Results ： The treatment was similar between the two groups is small, no statistical significance （ P 〉 0.05 ）. Rehabilitation treatment group patients were followed up for 12 months, there is a shoulder discomfort in 5 cases, after stretch limited has a 3 cases, limited forward bends in 1 case; In 2 cases of surgery in the treatment group after stretch restricted movement, under the intense activity in patients with the front of the shoulder discomfort after 2 cases, other clinical symptoms disappear. Conclusion ： Ttraumatic shoulder joint instability before the patient＇s rehabilitation treatment has good effect, but need a long time to recover, and will be affected by patients＇treatment compliance, articular damage, the influence of such factors as before treatment failure of patients require surgical treatment, in order to promote the recovery of joint function.
