参松养心胶囊对陈旧性心肌梗死患者自主神经功能的影响

摘　　要：目的观察参松养心胶囊对陈旧性心肌梗死患者心脏自主神经功能的影响。方法将104例陈旧性心肌梗死患者随机分为2组。治疗组52例予参松养心胶囊治疗,对照组52例予谷维素治疗。2组均治疗4周后统计临床疗效,并观察2组治疗前后焦虑自评量表(SAS)和抑郁自评量表(SDS)评分、正常RR间期标准差(SDNN)、平均值的标准差(SDANN)、标准差的平均值(SDANN index)、差值均方的平方根(RMS SD)、差值>50 ms的百分比(PNN50)及QT间期离散度(QTd)。结果治疗组总有效率90.38%,对照组总有效率65.39%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组治疗后SAS、SDS评分均降低(P<0.05),且治疗组降低优于对照组(P<0.05)。2组治疗后SDNN、SDANN、SDANN index均升高(P<0.05),RMS SD、PNN50均降低(P<0.05),且治疗组治疗后升高SDNN、SDANN、SDANN index,降低RMS SD、PNN50均优于对照组(P<0.05)。结论参松养心胶囊治疗陈旧性心肌梗死可以通过抑制迷走神经张力和相对提高交感神经张力来恢复自主神经系统的平衡,提高患者的心率并改善症状。Objective To observe the effects of Shensong- yangxin capsules on autonomic nervous function in patients with old myocardial infarction（ OMI）. Methods 104 subjects with OMI were randomly divided into two groups. 52 subjects in treatment group were treated by Shensong- yangxin capsules. 52 subjects in control group were treated by oryzanol. The clinical effect was evaluated 4 weeks after treatment in two groups. The self- rating anxiety scale（ SAS）,self- rating depression scale（ SDS）,the indices standard deviation of all NN intervals（ SDNN）,index of standard deviation of the averages of NN intervals（ SDANN）,mean of the standard deviation of all NN intervals performed on all 5- minute segments of the entire recording（ SDNN index）,square root of the mean of the sum of the squares of differences between adjacent NN intervals（ RMS SD）,NN50 count divided by the total number of all NN intervals（ PNN50） and QT discrete（ QTd） before and after treatment were observed in two groups.Results The total effective rate in treatment group（ 90. 38%） was superior to that in control group（ 65. 39%,P〈0. 05）. The SAS and SDS after treatment were decreased in two groups（ P〈0. 05）,and the increase in treatment group was superior to that in control group（ P〈0. 05）. The SDNN,SDANN,SDNN index after treatment were increased in two groups（ P〈0. 05）,and the RMS SD,PNN50 were decreased（ P〈0. 05）,the improvements in treatment group were superior to those in control group（ P〈0. 05）. Conclusion Shensong- yangxin capsules can recover autonomic nervous system function through the inhibition of vagus nerve activity and increased of sympathetic nerve system activity,which has treatment effects on OMI,can increase the heart rate and improve the symptoms.
