摘　　要：患者男,31岁,养羊史。上唇部、下颌部、颏部、颈前上部皮损4个月,逐渐加重。皮损呈红色肿胀性浸润性损害,上有结节、脓疱、结痂,有脓液外溢,压痛明显,胡须大部分脱落,亦见一些断须,易拔出。脓液真菌镜检可见透明分支分隔菌丝,真菌培养为指(趾)间毛癣菌,组织病理显示为感染肉芽肿改变。诊断须癣,给予患者口服伊曲康唑400 mg/d治疗6周,之后伊曲康唑200mg/d联合特比萘芬250mg/d治疗6周痊愈。A 31-year-old goat farmer presented with aggravating lesions on his upper lip,lower jaw,cheek and the front of the neck for 4months.Physical examination showed red swelling plaques,with marked nodules,pustules,pyorrhea and crusts on them and obvious tenderness.Most beards were off,with a few broken beards left and easy to pull out.Direct microscopic examination of pus showed septate hyaline hyphae with branches.Trichophyton interdigitale was isolated and identified from pus cultures.And histopathological features showed granulomatous changes.The diagnosis of tinea barbae caused by T.interdigitale was made based on the above findings.The patient was successfully treated with oral itraconazole 400 mg/d for 6weeks,followed by the combination of oral itraconazole 200mg/d and terbinafine 250mg/d for another 6weeks.
