摘  要:目的 了解柳江河镉污染后沿岸人群的镉性肾早期损伤情况。方法 根据《环境镉污染健康危害区判定标准》确定暴露区(暴露组)和对照区(对照组),随机抽查年龄在25~54岁长期定居居民,暴露组为166人,对照组186人,采集两组人群尿样,检测尿镉、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、β2-微球蛋白(β2-MG)、肌酐(Cr)水平。结果 暴露组尿镉中位数为0.30μg/g Cr,与对照组的0.33μg/g Cr比较,差异无统计学意义(P〉0.05);暴露组尿β2-MG中位数为91.80μg/g Cr,明显高于对照组的48.41μg/g Cr(P〈0.05);暴露组尿NAG中位数为6.65 U/g Cr,明显高于对照组的3.12 U/g Cr(P〈0.05)。暴露组尿镉、NAG、β2-MG 3项指标均达阳性判定值的病例数为0,联合反应率为0。结论 柳江河镉污染对沿岸人群的肾损伤存在潜在的风险。Objective To investigate the condition of early cadmium-induced renal injury in residents living along Liujiang river after cadmium pollution. Methods According to standard for assessment of health hazard area of environmental cadmium pollution, the exposure area( exposure group) and control area (control group) were determined. The permanent residents aged 25 -54 years were randomly selected. A total of 166 residents were enrolled in the exposure group and 186 residents in the control group. Urinary samples were collected from both groups, and then the levels of urinary cadmium, N-acetyl-beta-D-glucosaminidase (NAG) , beta 2-inicroglobulin (~32-MG) and ereatinine (Cr) were detected. Results No significant difference in the median level of urinary cadmium was observed between exposure group(0.30 μg/g Cr) and control group (0.33 μg/g Cr) (P 〉 0.05 ). The median 132-MG of exposure group was 91.80 μg/g Cr, and was significantly higher than that of control group (48.41 μg/g Cr, P 〈 0.05 ). The median NAG of exposure group (6.65 U/g Cr) was significantly higher than that of control group(3.12 U/g Cr,P 〈0.05 ). No ease reached the positive cut-off values of urinary cadmium, NAG and 132-MG concurrently in the exposure group,and the cooperation rate was O. Conclusion Potential risk for renal injury exists in the residents living along Liujiang river after cadmium pollntion.
