摘　　要：文章对美国以护士为主导的过渡期护理服务模式进行了详细介绍,旨在为我国过渡期护理服务模式的构建和发展提供借鉴与参考。通过该模式的构建为老年人提供从医院到社区或家庭的连续护理服务,并提高老年人的健康水平。In this paper, the Nurse-led transitional care mode of America is introduced in detail, which can provide reference for the development of transitional Care mode in our country. The mode can provide continuous nursing services from hospital to community or family for the elderly, and improve the health status of the elderly.
