基于因子分析与聚类分析的首发脑梗死急性期证候要素提取及证型研究

摘　　要：目的:探究首发缺血性脑梗死患者中医证候分布规律及特点。方法:运用SPSS 21.0进行因子分析和聚类分析。结果:本研究收集93个症状条目的KMO检验统计量为0.694,Bartlett的球形度检验近似卡方值为9 509.084(P<0.05)。应用主成分分析方法得到21个公因子,对病性相同的证候要素进行合并后得到6个公因子为风、火、痰、瘀、气虚、阴虚,聚类分析得到5大类中医证候:第一类为肝肾阴虚和气滞血瘀合而为病,第二类为脾肾阳虚和风火上扰合而为病,第三类为痰热腑实和心脾两虚合而为病,第四类为痰火瘀闭证,第五类为痰浊瘀闭证。结论:因子分析和聚类分析结合可以通过首发脑梗死急性期症状间的复杂关系,定性、定量化地凝练该病中医证候要素,为进一步研究该病提供工作基础与必要条件。Objective: To explore the distribution and characteristics of TCM syndromes in patients with primary ischemic cerebral infarction. Methods: Factor analysis and cluster analysis were performed by using SPSS 21.0. Results: In this study, the KMO test statistics for 93 symptom strips were 0.694, and the Bartlett's spherical degree test was 9 509.084(P<0.05). Application of principal component analysis method of 21 factors, merging the syndrome elements of disease after the same 6 factors as wind, fire, phlegm, blood stasis, qi deficiency, yin deficiency, the cluster analysis of 5 kinds of TCM syndromes of the first category for liver kidney yin deficiency and blood stasis and became a kind of disease. The second kind of disease was due to spleen and kidney yang deficiency and wind fire pathogen. The third kind of disease was due to phlegm heat and deficiency of both the heart and spleen. The fourth kind of disease was syndrome of obstruction of phlegm heat. The fifth kind of disease was syndrome of obstruction of phlegm turbidity. Conclusion: The combination of factor analysis and cluster analysis can not only contribute to the qualitative and quantitative concise of the disease of TCM syndrome factors but also provide basis and necessary conditions for the further study of the disease by the complex relationship of the symptoms of acute cerebral infarction.
