艾塞那肽联合二甲双胍对初诊成人2型糖尿病患者血糖控制效果观察

摘　　要：目的观察胰高血糖素样肽-1(GLP-1)受体激动剂——艾塞那肽联合二甲双胍在初诊成人2型糖尿病(T2DM)患者中的应用效果。方法对北京军区总医院内分泌科2013年11月—2015年6月诊治的78例T2DM的临床资料进行回顾性分析,依据治疗方法不同将78例分为二甲双胍单药组和艾塞那肽联合二甲双胍组,每组39例。二甲双胍单药组在饮食控制及运动锻炼的同时给予盐酸二甲双胍0.5 g口服、3/d;艾塞那肽联合二甲双胍组在二甲双胍单药组的基础上加用艾塞那肽注射液5μg皮下注射、2/d,1周后调整为10μg皮下注射、2/d。疗程为12周。观察比较两组治疗前后体重指数(BMI)、体重、空腹血糖(FBG)、餐后2h血糖(2 h BG)、糖化血红蛋白(Hb Alc)、血脂、血压、空腹胰岛素(FINS)的变化。结果两组体重、BMI及FBG、2 h BG、Hb A1c与治疗前比较显著下降,差异有统计学意义(P<0.05);艾塞那肽联合二甲双胍组甘油三酯(TG)、收缩压较治疗前显著下降,FINS较治疗前显著升高,差异均有统计学意义(P<0.05)。与二甲双胍单药治疗组比较,治疗后艾塞那肽联合二甲双胍组体重、BMI、FBG、2 h BG、Hb A1c、TG、收缩压显著下降,FINS显著升高,差异均有统计学意义(P<0.05)。两组均未发生明显不良反应。结论艾塞那肽联合二甲双胍可有效改善初诊成人T2DM患者的体重和糖代谢状态,且安全性较高。Objective To observe the effect of Exenatide [glucagon-like peptide-1（ GLP-1） receptor agonist]combined with Metformin in treatment of adult patients with type 2 diabetes mellitus（ T2DM） by preliminary diagnosis.Methods Clinical data of 78 patients with T2 DM by preliminary diagnosis during November 2013 and June 2015 was retrospectively analyzed,and the patients were divided into Metformin monotherapy group（ n = 39） and Exenatide combined with Metformin group（ n = 39） according to therapic methods.The Metformin monotherapy group was treated with0.5 g Metformin Hydrochloride orally（ 3 / d） based on alimentary control and exercise.Exenatide combined with Metformin group was injected with 5 μg Exenatide subcutaneously（ 2 / d） based on the treatment of Metformin monotherapy group,and then was adjusted to 10 μg of subcutaneous injection（ 2 / d）.The course of treatment was 12 weeks.The value changes of body mass index（ BMI）,body weight,fasting blood glucose（ FBG）,2 h postprandial blood glucose（ 2 h PBG）,and glycated hemoglobin（ Hb Alc）,blood fat,blood pressure and fasting serum insulin（ FINS） were observed and compared before and after treatment in the two groups.Results The values of body weight,BMI,FBG,2 h PBG and Hb Alc after treatment were significantly decreased compared with those before treatment in the two groups（ P〈0.05）;the values of triglyceride（ TG） and contractive pressure were significantly decreased,while the FINS value was significantly increased after treatment compared with those before treatment in Exenatide combined with Metformin group（ P〈0.05）.After the treatment,the values of body weight,BMI,FBG,2 h PBG,Hb A1 c,TG and contractive pressure were significantly decreased,while the FINS value was significantly increased in Exenatide combined with Metformin group compared with those in Metformin monotherapy group（ P〈0.05）.No obvious adverse reaction was found in the two groups.Conclusion Exenatide combined with Metformin can signi
