探索远程病理诊断在医疗援疆中的应用

摘　　要：目的 :通过在医疗支援新疆喀什地区泽普县的过程中利用远程病理诊断技术,探讨与总结在医疗援疆中应用远程病理诊断的可行性和存在的优缺点。方法 :利用闵行区对口支援新疆泽普县的契机,经中国数字病理远程诊断与质控平台(www.mpathology.cn)由复旦大学附属闵行医院病理科为泽普县人民医院病理科提供远程病理切片阅片和诊断。通过关键人物访谈了解援疆远程病理诊断的开展情况,并统计诊断开通以来至2013年12月的诊断病例数、各病例的取材部位和诊断结果等相关数据。结果 :泽普县人民医院与复旦大学附属闵行医院的远程病理诊断于2012年7月开通,至2013年12月,共诊断病例979例,确诊率91.4%。结论 :远程病理诊断应用于医疗援疆完全可行,通过应用远程病理诊断,泽普县病理科医师短缺的问题得到了解决;同时,此举为信息化援疆提供了新的思路,将来可有望广泛、长期应用于医疗援疆。Objective：To investigate the feasibility and advantages in applying telepathology to remote medical support in Xinjiang Uygur Autonomous Region through establishing telepathology access and providing pathology consultation between the People’s Hospital of Zepu County,Kashgar Region,Xinjiang and Minhang Hospital,Fudan University,Shanghai. Methods：Based on China digital pathology platform for remote diagnostics and quality control（www. mpathology.cn）,a key-informant interview was taken to investigate the construction of the telepathology access between the two districts and the functioning status of the pathology consultation. Then the statistical material was collected from July of 2012 to December of 2013. The number of cases and the sources from every biopsy specimen were counted and the rates of clear diagnosis during that period were also calculated. Results：The remote consultation was launched on July of 2012 and 979 cases had been consulted online until December of 2013. Among the 979 cases,91.4%were clearly diagnosed. Conclusion：It is entirely feasible to apply telepathology to remote medical support in Xinjiang Uygur Autonomous Region. The shortage of pathologist in Zepu County can be relieved through the distant pathology consultation. This method provides new ideas for the informatization of medical support in Xinjiang,and it is expected to be adopted widely and permanently in the remote medical support for the other remote parts in Xinjiang and China in the future.
