酚妥拉明治疗小儿肺炎心衰及改善肺部啰音的临床研究

摘　　要：目的探讨酚妥拉明治疗小儿肺炎心衰及对改善肺部啰音的临床疗效。方法选取2014年10月-2015年10月收治的肺炎合并心力衰竭患儿120例,采用随机简单数字表法将患儿分为对照组和观察组各60例。对照组患儿给予常规对症治疗,包括镇静剂、肾上腺皮质激素、利尿剂等药物治疗;观察组患儿在常规治疗的基础上加用酚妥拉明治疗,连续治疗一周后对两组患儿的临床疗效及肺部啰音改善情况进行比较。结果观察组治疗后的临床总有效率为95.0%,对照组治疗后的临床总有效率为73.3%,观察组疗效明显高于对照组,差异有统计学意义（P〈0.05）;观察组在心力衰竭控制时间、肺部罗音消失时间及喘息症状消失时间均短于对照组,差异均具有统计学意义（P〈0.05）;与对照组比较,观察组患儿治疗后其心肌酶水平值（CK-MB）明显降低,但射血分数（EF）与E/A值比较差异无显著性（P〉0.05）;治疗期间,观察组与对照组患儿均未出现明显的不良反应,组间比较差异无统计学意义（P〉0.05）。结论对小儿肺炎合并心力衰竭在常规治疗基础上加用酚妥拉明安全可靠能够起到良好的临床效果,并且能够同时改善患儿肺部啰音,缩短患儿住院时间,有利于患儿康复预后。Objective To investigate the curative effect analysis on treatment of pediatric pneumonia,heart failure and lung＇s improvement by phentolamine.Methods 120 cases of children pneumonia with heart failure were selected in our hospital from October 2014 to October 2015.The patients were randomly divided into control group（n=60）and observation group（n=60）.The control group was given conventional treatment,including a sedative,adrenal cortical hormone,diuretic drugs.The observation group was treated with phentolamine on the basis of given conventional treatment.The clinical effect and the lung sound improvement were observed.Results The total effective rate of observation group（95.0%）was obviously higher than that of control group（73.3%）（P〈0.05）.The heart failure,lung rale disappearing time and breathing symptoms disappear time of observation group were significantly reduced than that of control group（P〈0.05）.Compared with that of the control group,myocardial enzyme levels of observation group after treatment（CK-MB）significantly decreased（P〈0.05）.The ejection fraction（EF）and E/A value of the two groups were no significant difference（P〉0.05）.During treatment,the adverse reactions of observation group and control group had no statistical significant differences（P〉0.05）.Conclusion For pediatric pneumonia with heart failure,phentolamine can significantly improve children＇s lung sound,shorten the time of children in the hospital and is advantageous to the prognosis of children with recovery.
