腹腔镜全子宫与次全子宫切除术对患者卵巢及其性功能的影响

摘　　要：目的探讨腹腔镜全子宫切除术和腹腔镜次全子宫切除术对患者卵巢功能及其性功能的影响。方法选择2012年6月至2015年6月在我院接受手术治疗的子宫肌瘤患者144例,根据患者病情及手术指征,将患者分为观察组和对照组各72例,观察组给予腹腔镜次全子宫切除术治疗,对照组给予腹腔镜全子宫切除术治疗,对比两组患者手术时长、出血量、肛门排气时间、住院时间及手术前后的卵巢功能和不同时期的性功能。结果观察组手术时长、出血量、肛门排气时间以及住院时长均分别显著少于对照组,差异均有统计学意义(P<0.05)。手术后3个月两组的E2水平较术前均显著下降,但观察组的E2水平显著高于对照组,两组FSH水平较术前均显著上升,但观察组的FSH水平显著高于对照组,差异均有统计学意义(均P<0.05)。两组术后1个月及术后3个月的Olson性功能评分均分别显著高于术前,且观察组分别显著高于对照组,差异均有统计学意义(均P<0.05)。结论腹腔镜次全子宫切除术对子宫肌瘤患者的卵巢功能与性功能的影响较小,安全性较高,在患者病情安全可控的前提下应优先考虑此种术式,可在临床推广应用。Objective To study effect of laparoscopic total hysterectomy and laparoscopic subtotal hysterectomy on ovarian function and sexual function in patients.Methods According to the patient＇s condition and the operation indications,144 patients were divided into observation group（72cases）and control group（72cases）.The observation group received laparoscopic subtotal hysterectomy.The control group was given total laparoscopic uterine resection.The operation time,amount of bleeding,anal emptying time,hospitalization time,ovarian function and sexual function were observed.Results The length of operation,bleeding volume,anal exhaust time and length of hospital stay of observation group were significantly less than that of the control group.The differences were statistically significant（P〈0.05）.E2 compared significantly decreased after operation.E2 of the observation group was significantly higher than that of the control group.FSH of the two groups significantly increased after operation,FSH of observation group was significantly higher than those in the control group.The differences were statistically significant（P〈0.05）.Olson score of the observation group was significantly higher than that in the control group.The difference was statistically significant（P〈0.05）.Conclusion Laparoscopic subtotal hysterectomy operation is not affected on patients with uterine fibroids ovarian function and sexual function and has high security.
