双歧杆菌活菌联合蒙脱石散治疗小儿腹泻疗效观察

摘　　要：目的探讨双歧杆菌活菌联合蒙脱石散治疗小儿腹泻的疗效及对患儿免疫功能的影响。方法按照入选标准选取126例小儿腹泻患儿,随机分为对照组和观察组各63例。两组患者均给予静脉补液、降温、纠正电解质紊乱等常规治疗,对照组患者在常规治疗的基础上给予口服蒙脱石散治疗,观察组患者在对照组的基础上加用双歧杆菌活菌。治疗3天后观察两组患者的治疗效果及免疫功能相关指标情况。结果观察组总有效率(92.1%)明显高于对照组总有效率(73.0%),差异具有统计学意义(P<0.05)。观察组患儿症状缓解时间,包括退热时间、腹痛缓解时间、大便恢复正常时间、呕吐缓解时间、大便常规恢复正常时间等均显著短于对照组,差异均有统计学意义(均P<0.05);免疫功能指标检测显示,治疗后两组患儿的IgA、IgG、CD4/CD8比值均有不同程度的升高,IL-6、TNF-α均有不同程度的降低,与治疗前相比均有统计学意义(均P<0.05)。与对照组治疗后比较,观察组治疗后IgA、IgG、CD4/CD8比值明显升高,IL-6、TNF-α明显降低,差异均有统计学意义(均P<0.05)。两组患儿在治疗过程中均未出现严重不良反应。结论双歧杆菌活菌联合蒙脱石散对小儿腹泻有较好的临床疗效,可快速缓解症状,有效控制病情,并能提高患者免疫功能,不良反应少,值得临床进一步研究探讨。Objective To investigate the clinical effect of viable bifidobacterium combined with montmorillonite powder on treatment of infantile diarrhea and its effects on immunological function.Methods 126 children with infantile diarrhea were enrolled in this study according to the inclusion criteria.They were divided into observation group and control group randomly.All the cases were given conventional treatment such as cooling,intravenous fluids,correcting electrolyte disorder,then children in control group were given montmorillonite powder,and children in observation group were add viable bifidobacterium based on the control group.The therapeutic effect and effect on immunological function after 3days＇treatment were observed.Results The total effective rate of observation group was significantly higher than that of control group（92.1% vs.73.0%,P〈0.05）.The fever clearance time,remission time of abdomen pain and vomit,normalization time of defecating and stool routine examination of observation group were obviously shorter than that of the control group（P〈0.05）.The level of IgA,IgG and CD4/CD8 ratio of both two group after treatment were higher than that before treatment（P〈0.05）.The level of IL-6and TNF-αof the two group after treatment were lower than that before treatment（P〈0.05）.The level of IgA,IgG and CD4/CD8 ratio of observation group were significantly higher that of the control group（P〈0.05）and the level of IL-6and TNF-αof observation group after treatment were significantly lower（P〈0.05）that of control group after treatment.There were no serious adverse reactions in both two groups.Conclusion Viable bifidobacterium combined with montmorillonite powder is effective on treatment of infantile diarrhea,which may relieve symptoms quickly,control symptom effectively,improve immune function and have less adverse reactions.
