分娩方式、体重指数和新生儿出生体重对女性盆底功能障碍性疾病的影响

摘　　要：目的探讨不同发病因素对女性盆底功能障碍性疾病(pelvic floor desfunction,PFD)的影响。方法选择在2014年1月至2015年6月于我院就诊的150例女性。比较不同分娩方式、体重指数及新生儿体重的女性盆底反馈电刺激各项指标以及压力性尿失禁(SUI)组和对照组盆底超声指标的不同,采用多因素Logistic回归分析确定女性PFD的影响因素。结果盆底反馈电刺激评估中的104例女性三种不同分组方式组间最大收缩力和持续收缩力比较,差异有统计学意义(P<0.05)。盆底超声测定的111例妇女,SUI组膀胱颈角度、膀胱旋转角度及膀胱颈移动度等明显高于正常组,两组比较差异有统计学意义(P<0.05)。Logistic回归分析显示,分娩方式、体重指数、新生儿出生体重等是SUI的独立危险因素(P<0.05)。结论分娩方式、体重指数、新生儿出生体重是PFD的独立显著影响因素,盆底超声测定与盆底反馈电刺激评估盆底功能在PFD中诊断中具有重要的临床意义。Objective To explore the effect of different pathogenic factors on pelvic floor functional disorder.Methods A randomized method was used to select the 150 patients,who were treated in our hospital from January,2014 to June,2015.The analysis of variance was used to compare the various indicators of pelvic feedback electrical stimulation among three groups,and analyze the difference of pelvic ultrasound indicators between SUI and control groups. The relative independent factors for pelvic floor dysfunction（ PFD） were analyzed by using multiple logistic regression analysis.Results In 104 patients undergoing the pelvic floor muscle tissue assessment,a significant difference was found in the biggest contraction force and sustained contraction force of pelvic floor muscle tissue among the three groups（ P〈 0. 05）.In 111 cases undergoing the pelvic ultrasound measurement,the bladder neck angle,the rotation angle of the bladder and the bladder neck mobility in the SUI group were significantly higher than that in the control group（ P〈 0. 05）.The Logistic regression analysis showed that the mode of delivery,BMI and birth weight were all independent factors for SUI（ P =0. 000,0. 003 and 0. 035,respectively）.Conclusion The mode of delivery,BMI and birth weight is all the independent influence factors for PFD.The pelvic ultrasound and pelvic floor feedback electrical stimulation have important clinical implications in the diagnosis of the PFD.
