羟乙基淀粉130/0.4在腹部大手术后的应用

摘　　要：目的观察羟乙基淀粉130/0.4液体治疗对腹部大手术后的患者液体平衡、凝血功能、肾功能的影响。方法2013年1月至2014年6月行急诊腹部手术的患者90例,简单随机法分为羟乙基淀粉组和晶体组各45例。晶体组使用常规晶体液治疗,羟乙基淀粉组给予羟乙基淀粉130/0.4加晶体液治疗。比较两组患者术后1~3天液体平衡量、血浆凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）及肌酐（Cr）。结果两组患者在术后1~3天均存在液体正平衡,且晶体组液体正平衡量显著高于羟乙基淀粉组（P〈0.05）。两组患者术后PT、APTT及CR比较,差异无统计学意义（P〉0.05）。结论适量的人工胶体羟乙基淀粉130/0.4液体治疗能显著减少腹部大手术后患者术后液体正平衡和水钠潴留,且对凝血功能和肾功能无明显影响。Objective To observe the effects of hydroxyethylstarch 130/0. 4 liquid on fluid balance,blood coagulation function and renal function in patients after abdominal operation.Methods Ninety patients who underwent emergency abdominal operation from January,2013 to June,2014 were randomly divided into hydroxyethylstarch or crystal group,45 in each group. The crystal group was treated with conventional crystal liquid treatment while the hydroxyethylstarch group was given hydroxyethylstarch 130 / 0. 4 intravenous drip plus crystal liquid treatment.The amount of fluid balance,blood coagulation function and renal function were compared between the two groups after 1 ~ 3 days of operation.Results There was a positive fluid balance in the both groups after 1-3 days of operation,and the positive fluid balance in the crystal group was significantly higher than that in the hydroxyethylstarch group（ P〉 0. 05）. There was no significant difference in the PT,APTT and CR between the two groups（ P〈 0. 05）.Conclusion Certain amount of artificial colloid hydroxyethylstarch 130 / 0. 4 liquid can significantly decrease the postoperative positive liquid balance and water sodium retention in patients after abdominal operation.This method has no significant influence on blood coagulation and renal function.
