摘 要:目的:观察芪芍胶囊对糖尿病肾病(diabetic nephropathy,DN)模型大鼠血糖、DN相关指标的影响,探讨其肾脏保护作用机制。方法:采用单侧肾脏切除和腹腔注射链脲佐菌素的方法建立大鼠DN模型,正常大鼠分为正常组,模型大鼠随机分为随机模型组、芪芍胶囊组(1 g·kg^(-1))和缬沙坦组(2 mg·kg^(-1))。首次给药时,进行大鼠口服糖耐量实验,给药8周后观察各组大鼠糖化血红蛋白(glycosylated hemoglobin,Hb A1C)、血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum Scrtinine,Scr)、血清胱抑素C(serum cystatin C,Cys C)和24 h尿蛋白定量,并进行肾脏组织病理学评价,并通过ELISA检测肾脏组织中的转化生长因子β1(transforming growth factor beta 1,TGF-β1)与结缔组织生长因子(connective tissue growth factor,CTGF)的表达。结果:在口服糖耐量实验实验中,芪芍胶囊显著抑制给予糖负荷后的大鼠血糖升高;治疗8周后,模型组大鼠血清BUN、SCR、Hb A1c、Cysc和24小时尿蛋白定量都显著高于正常组(P<0.05);与模型组比较,芪芍胶囊显著降低上述指标(P<0.05);缬沙坦组显著降低BUN、SCR、Cysc和24小时尿蛋白定量(P<0.05),血清Hb A1C与模型组比较,差异无统计学意义(P>0.05)。模型组大鼠肾小球基底膜弥漫性增厚,系膜轻中度增厚,肾小管扩张,肾间质存在炎性细胞和淋巴细胞浸润;芪芍胶囊可以显著改善肾脏病理等级,且与缬沙坦组相比,无明显差异(P>0.05)。ELISA检测结果表明,芪芍胶囊可以显著降低大鼠肾脏中的TGF-β和CTGF的表达。结论:芪芍胶囊可以显著改善DN大鼠血糖水平,对肾脏有保护作用,能够降低DN大鼠肾脏中TGF-β和CTGF蛋白水平,可能是芪芍胶囊肾脏保护作用机制。Objective: To observe the effectiveness of Shao Qi Capsule on model rat with diabetic nephropathy in blood sugar level and other related indicators and explore the mechanism. Methods: Rat model with diabetic nephropathy was made by unilateral nephrectomy and intraperitoneal injection of streptozotocin. Normal rats was used as the normal group while the modeled rats were randomly divided into model group,Qi Shao Capsule group and valsartan group. 8 weeks later after treatment,the rats' glycosylated hemoglobin( Hb A1C),blood urea nitrogen( BUN),serum creatinine( SCR),serum cystatin C( Cys C),24-hour urinary protein excretion,renal histopathological evaluation,transforming growth factor beta 1( TGF-β1),connective tissue growth factor( CTGF)expression were measured. Results: after 8 weeks of treatment,the Hb A1 C,BUN,SCR,Cys C,24-hour urinary protein of the model group were significantly higher than those of the normal group( P〈0. 05). Compared with the model group,Qi Shao Capsule significantly reduced the index( P〉0. 05). The BUN,SCR,Cysc and 24-hour urinary protein in the valsartan group were significantly reduced( P〈0. 05),while compared with model group,the difference of Hb A1 C was not statistically significant( P〈0. 05).Shao Qi Capsule can significantly reduce the expression of TGF-β and CTGF in rat's kidney. Conclusion: Qi Shao Capsule can significantly improve blood sugar levels in DN rats and has a protective effect on the kidney by reducing TGF-β and CTGF protein levels in rat kidney DN.
