摘  要:目的:观察托里透脓汤联合氦氖激光局部照射治疗小腿骨皮缺损皮瓣修复术后感染的临床疗效。方法:将60例患者随机分为对照组和治疗组,每组30例。对照组入院后在常规外科换药基础上,第1天开始使用低功率氦氖激光照射感染伤口,每日2次;治疗组在对照组基础上加用托里透脓汤(穿山甲3 g,皂角刺、麸炒白术、白芷、甘草、升麻、青皮各6 g,党参、当归各9 g,黄芪30 g)治疗,每日1次。治疗后比较患者的临床疗效、痊愈时间、抗菌素使用时间以及分泌物耐药菌株情况。结果:两组患者临床疗效比较,差异有统计学意义(P<0.05);两组患者痊愈时间比较,差异有统计学意义(P<0.05);两组患者抗菌素使用时间以及分泌物耐药菌株情况比较,差异有统计学意义(P<0.05)。结论:托里透脓汤联合氦氖激光局部照射能改善局部血液循环、杀灭病原微生物、加速感染创面愈合,且能脱毒排脓。Objective: To observe the clinical efficacy of Tuoli Tounong Decoction combined with helium-neon laser therapy in treatment of infection on leg skin defect. Methods: 60 patients were randomly divided into control group and treatment group,with 30 cases in each group. The control group,after routine surgical dressing,low-power helium-neon laser irradiation was conducted on the first day of infection of the wounds twice daily; treatment group were treated with Tuoli Tounong Decoction( Pangolin 3 g,Soap nail,Bran fried Atractylodes,Angelica,Licorice,Black cohosh,Blue each 6 g,Codonopsis and Angelica 9 g,Astragalus 30 g) besides that with the control group once daily. After treatment,the clinical efficacy,the recovery time,the use of antibiotic resistant strains secretions time and circumstances were compared. Results: The differences of the clinical efficacy,patients' cured time,time of patients using antibiotic resistant strains secretions conditions were statistically significant( P〈0. 05). Conclusion: Tuoli Tounong Decoction combined with helium-neon laser therapy in treatment of infection on leg skin defect can improve local blood circulation,kill pathogenic microorganisms,accelerate the healing of infection wound,and detoxify and dispel abscess.
