中西医治疗老年性痴呆研究述评

摘　　要：老年性痴呆又称阿尔茨海默病,是因各种原因导致的大脑功能障碍而引发的获得性及持续性智能损害综合征。临床表现主要为记忆力减退、定向不准确、判断能力丧失、语言表达能力丧失、缺乏逻辑判断能力、意识模糊、人格分裂和情感改变等认知功能发生慢性和持续性障碍,个别患者还伴有脾气暴躁、精神抑郁、紧张感等行为。中医具有几千年的痴呆治疗经验,理论体系成熟,在AD的治疗上更具优势,但中医辨证施治过程中有些问题仍需重视加以解决,如辨证不统一,有医家从补肾填精论治出发,兼顾化痰开窍或活血行血;有医家从痰瘀虚论治,以补肾化痰祛瘀为治;还有医家从毒损脑络立论,以祛毒通络为治,等。疗效评估标准不一,缺乏大样本系统研究及中西医结合研究,均制约了中医在AD治疗上的优势发挥。对本病的治疗应充分发掘、发挥中医优势,深入研究本病的病因病机及有效治疗方案,采用中西医协同治疗,统一疗效评估标准,建立大样本临床研究,寻求更加安全有效的治疗方法,争取在AD的治疗上取得一定的进展和突破。Senile dementia,which is known as Alzheimer＇ s disease,is a kind of acquired and persistent intellectual impairment syndrome with brain dysfunction due to many factors. The main clinical manifestations include chronic and persistent obstacles in cognitive function such as memory loss,inaccurate orientation,the loss of ability to judge,loss of language skills,lack of logic judgment,confusion,split personality and emotional changes. Some patients also suffer from irritability,depression,tension and others.Traditional Chinese medicine has thousands of years＇ experiences in treatment of dementia with mature theoretical system and more advantages in the treatment of AD. However,in Chinese medicine syndrome differentiation process and the clinical efficacy are not standard,which restricts the advantages of Chinese medicine in treatment of AD. There is also lack of large sampled systematic integrative medical treatment studies. Therefore,the treatment of this disease should include the in-depth studies in pathogenesis and the effectiveness by using synergistic therapeutic methods of Chinese and western medicine with unified assessment criteria and large scale clinical trials to seek more effective and safe treatment and to make further progress in treatment of AD.
